DAMAGE INSPECTION REPORT

DATE OF REPORT: 22/12/2016

APPLICANT: WALLEM SHIPPING HAIPHONG

VESSELS NAME: M/V VICTORIA HARBOUR

PLACE OF INSPECTION: DA NANG PORT, VIETNAM

At the request of WALLEM SHIPPING HAIPHONG ; We - surveyor of


VINACONTROL DA NANG present at the M/V VICTORIA HARBOUR at 0600 hrs on
22/12/2016 for damage inspection. We found that:

According to Master, at 0820hrs on 06th DEC., 2016 during the opening of all
cargo holds for checking condition of cargo, founded that the lashing wire were broken,
the equipments moved and damaged in cargo hold No3.

At 0420 hrs on 22ND, DEC. 2016 the M/V VICTORIA HARBOUR berth at the jetty
No4 Danang PORT.

0510hrs vessel safety side alongside.

From 0630hrs to 0730hrs 22ND, DEC. 2016 opening hatches cover, preparing for
discharging.

Before discharging, we observed that one wooden box of cargo in cargo hold
No3 was broken. Compared to pictures of cargo at loading port supplied by Chief
Officer, the wooden boxes of the cargo were shifted away from the original location, and
the lashing wire were replaced by chains.

0810hrs 22ND, DEC. 2016 commenced discharging cargo.

After checking and taking the pictures of related evidences in the cargo hold and
at the time of discharging, we described the damage as follows( pictures attached):

- The wooden box of package XKX-HCN-010-Axiang-002 in normal condition

- The wooden box of package XKX-HCN-010-Axiang-001 was broken on one


side

- The wooden box of package XKX-HCN-010-SE-007 was heavily broken on all


sides.

- The upper parts of packages XKX-HCN-010-SE-001 and XKX-HCN-010-SE-


002 were dented.

- The package XKX-HCN-010-SE-008 was slightly scratched.


- All other cargo in cargo hold No4 in normal condition.

Basing on what we surveyed and information available, it is our opinion that:

-Initially, the wooden boxes with cargo insde were bound in cargo hold with
lashing wire. But during the voyage from Tianjin, China to Bayuquan, China under rough
sea condition and wind scale 7(Ships log book), the heavy cargo moved and the
lashing wire system couldnt stand the tension and was broken. After, the lashing wire
were replaced by chain.

-The cargo arrangement and method of lashing in this case is inappropriate.


When the lashing wire split, the movement of two cargo boxes on the left side squeezed
the damaged cargo box and led to the occurrence of the incident. Instead of using wire,
the stevedore can use chain to fasten the heavy cargo and keep them into position.

-The lashing of the steel pipes under the damaged cargo box in normal condition.

The reason and extent of the damage will be determined afterwards.
