wherein she sustained a minor laceration.
 Upon palpation, Nurse Antonette noticed a slight
bulging on the left parietal part of her head but Ms.
Caronongan stated that the pain was tolerable. Nurse
Antonette advised the patient to seek immediate
medical attention for evaluation, clean the wound to
avoid the risk of infection and apply cold compress for
the first 48hrs. Nurse Antonette offered a mobile
assistance to transport her to the hospital for further
assessment but the patient refused therefore Nurse
Antonette let the patient signed the waiver and
quitclaim in the presence of Mr. Jun Rey Buyco of
Housekeeping dept.
