Lordstown Care Plan: Decreasing Infant Mortality

Healthy People 2020 Goal:


A Healthy People 2020 goal is to improve the health and well being of women, infants,
children and families. According to Healthy People, there was an average of 6.7 infant
deaths per 1,000 live births in 2006. Their target range is 6.0 infant deaths per 1,000 live
births. The objective MICH 1.3 is to reduce the rate of all infant deaths within 1 year.
(Healthy People 2020).

Short-Term Goal:
In 2015, there were 20 infant deaths per 2,079 births in Trumbull County. That creates a
9.6 ratio per 1,000 births (odh.ohio.gov). This ratio is 3.6 higher than the Healthy People
2020 goal of 6.0. To help lower these numbers, programming is needed to inform families
of ways to reduce infant mortality. A short-term goal is to establish a partnership with
Mercy Health System to educate pregnant women on available resources.

Long Term Goal:


- Decreases infant mortality by 25% within the first five years
- Pregnant women and families of Lordstown will have improved health and
quality of life.

Outcome Criteria:
- Within three months, a committee is to be formed with the City Council to discuss
the important of improving the overall health of families: focusing on pregnant
mothers and infant children of Lordstown.
- Within six months, awareness of the high mortality rate throughout Lordstown.
Citizens will desire to improve the heath of families, pregnant mothers, and
infants.
- Within eighteen months, the committee and Mercy Health will be taking part in
National Campaigns, such as Celebrate Day 366, to raise awareness and
education.
- Within twenty-four months, the infant mortality will be dropped to within range
of the Healthy People 2020 goal of 6.0.

Nursing Diagnosis:
Increased risk of infant mortality in Lordstown Community related to lack of knowledge
regarding prenatal care.

Evidenced by:
- 20 infants died before their first birthday in 2015, according to Ohio Department
of Health. (Odh.ohio.gov)
- According to Trumbull County website, their goal is to reduce the infant mortality
rate to the Healthy People 2020 goal of 6.0 per 1,000 deaths.
- Lack of prenatal programs for pregnant women and families (trumbull.oh.org).

Interventions/ rationales:

- Partnering Mercy Health with the CDC to require public health agencies to work
together with health care providers, communities, and partners to reduce infant
deaths in the United States (cdc.gov).
- Participating in Celebrate Day 366 Campaign in September to make families
aware of government policies and programs to help reduce infant mortality rates.
- Participation in the public Back to Sleep campaign to educate parents and
caregivers that babies who sleep on their backs have a lower risk of SIDS
(http://hhs.gov).
- Educate about the importance of prenatal care and possible risks to client and
fetus to reinforce the ongoing assessment crucial to health and well being of
client and fetus (Lowdermilk, Perry, Cashion, Alden, 2012, pg. 361)
- Assess diet history to determine prescriptions for additions or changes in present
dietary pattern (Lowdermilk, Perry, Cashion, Alden, 2012, pg. 361)
- Educate about risks of alcohol, tobacco, and illicit and prescription drug us to
minimize risk to client and fetus (Lowdermilk, Perry, Cashion, Alden, 2012, pg.
361)

Three-month evaluation:
- By the end of three months, a committee will be formed and discussing the
importance of reducing Lordstown infant mortality rate.
- By the end of six months, the community has increased awareness of the high
mortality rate. Citizens desire to make improvements in the health of other
community members.
- By the end of eighteen months, Mercy Health and the Lordstown community are
partaking in government campaign, Celebrate Day 366, to increase awareness.
- By the end of twenty-four months, the infant mortality rate has dropped within
target range.
