JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, LITO A. CONCEPCION, of legal age, Filipino citizen, and a

resident of Gomez St., Poblacion A, Camiling, Tarlac and MIGUEL G.
TIAMZON, of legal age, Filipino citizen, and a resident of Burgos St.,
Poblacion A, Camiling, Tarlac, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. We both personally know NOMIE ESPERANZA

LORENZO REYES (Nomie Esperanza, for brevity),born on March
28, 1957 to Spouses Jesus B. Reyes and Charity Lorenzo in
Camiling, Tarlac;

2. NomieEsperanze is also known as MRS. NOMIE

ESPERANZA REYES DOCTOR after she got married to Prudencio
C. Doctor;

3. Nomie Esperanza and Prudencio were blessed with five

(5) children, namely: Giovannie, Joanna Marie, Kathleen Grace,
Princess May and Prince PJ, all surnamed Doctor;

4. When Nomie Esperanza secured copies of her Certificate

of Birth, Marriage Contract, and the Certificate of Live Birth of her
son Prince PJ Reyes Doctor from the Philippine Statistics Authority
(PSA) / National Statistics Office (NSO), and after scrutinizing those
documents, she discovered that her name appearing in her
Certificate of Birth is NOMIE ESPERANZA M. REYES, instead of
NOMIE EZPERANZA L. REYES as recorded in her Marriage
Contract;

5. While in the Certificate of Live Birth of Prince PJ, the

recorded name of Nomie Esperanza is NOIME LORENZO REYES,
instead of NOMIE ESPERANZA LORENZO REYES. Hence, the
discrepancies;

6. We are not related to Nomie Esperanza by affinity or

consanguinity, except that of being herbarangay mates;

7. We are executing this Affidavit to attest to the fact that

the names NOMIE ESPERANZA M. REYESas recorded in
herCertificate of Birth, NOMIE ESPERANZA L. REYES as recorded
 in her Marriage Contract, NOIME LORENZO REYES as recorded in
the Certificate of Live Birth of Prince PJ Reyes Doctor, and MRS.
NOMIE ESPERANZA REYES DOCTOR, refer to one and the same
person,and that is the wife of Prudencio C. Doctor, the biological
mother of Prince PJ Reyes Doctor, our barangay mate;

8. Finally, we are executing this Affidavit to attest to the

truth of the foregoing statements and for all legal intents and
purposes it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures
