V. DISTRICT-WIDE POLICIES:
Students who have a password from a prior semester will be able to login using their old
credentials. Students who do not have an established password or who have forgotten
their password should go to the website, select the Forgot Your Password link, enter
their FSW username as the Username, and click on continue. An email with a link for
resetting the password will be sent from support@pearson.com to the students FSW
email account
ACCESS CODES: After logging in and selecting this course, students will be
prompted with three enrollment options to gain access to their course:
VPAA: Revised 9/11, 3/14, 11/16 Page 3
MAT 1033 INTERMEDIATE ALGEBRA
o Students will complete all on-line homework assignments posted in MMLP; see Section XI: Class
Schedule. The due date for each assignment will be found in MMLP.
o Students should print a copy of the assignment and work out all problems with pencil and paper.
Work should be neatly organized and should show steps in the solution process rather than just
answers. Homework should be stored in a notebook dedicated to the course.
o The student should complete each assignment by its due date. In most cases, the student will be
able to submit all or part of an assignment late until the day of the test covering that material;
however there will be a 10 % penalty applied to all problems done after the deadline.
o For any assignment not attempted you will receive a grade of 0.
o Your homework grade will make-up 10% of your overall grade.
TESTS:
o Students will take a total of three (3) tests as scheduled; see Section XI: Class Schedule. Students
are expected to take all three tests. If you miss one test, your final exam grade will be used for
that grade. If you miss more than one test, all other tests will receive a 0. If you take all three
tests, your lowest grade may be replaced with your final exam grade.
o The second and third tests are proctored exams and must be taken at one of the FSW testing
centers or other approved testing facility. (See the paragraph on proctored exams below.) Students
will have an hour and 40 minutes to complete a test. Only one attempt is allowed and tests will not
be accepted late. You must schedule the second and third tests with the testing site, giving yourself
enough time to complete the test before the center closes.
o Any test not taken will receive a grade of 0.
o The first non-proctored test counts for 15% of the overall grade. The second and third proctored
tests each count 20% of the overall grade.
FINAL EXAM:
The final exam is mandatory and it will be proctored. It must be taken at one of the FSW State testing
centers or some other approved testing facility. The dates that the exam may be taken are listed in
Section XI: Class Schedule. Students will have 1 hr. 50 minutes to complete the final exam. Its
located in Canvas and can be accessed by clicking on Modules and then by scrolling down to the last
module.
Good Study Skill: Inform the instructor of any problems you may have with the material
a few days before the testing date. You should work hard on the homework and quizzes
and continually ask questions. In this way, the tests become challenging without being
frightening.
o Your final exam grade will make-up 20% of your overall grade. Failure to take the Final Exam will
result in receiving a 0 for the exam and may cause you to fail the course.
PROCTORED EXAMS:
o To take all the tests and the final exam students must go to one of the FSW State Testing Centers
or some other approved testing facility (during the dates posted in section XI of the syllabus.)
o Information about the Florida SouthWestern State Testing Centers may be found at
http://www.fsw.edu/online/testing. To receive approval for an alternate testing center, read the
information on proctored exams found at http://www.fsw.edu/online/testing/proctoredexams
To use an alternate testing center, students must complete and submit a Proctored Exam Form
at least 3 business days prior to the date of the exam.
MAKE-UP POLICY:
Make-ups will not be given. Please refer to the test dates listed in section XI and plan accordingly. If a
student misses a test, then the percentage grade of the Final Exam will be counted in place of one missed
test, if more than one test is missed they will remain zeros. If a student has taken all of their tests, then
their final exam score can replace their lowest test score.
RESPECT:
Students are expected to treat each other and the professor with the utmost respect. Flaming will not
be tolerated. What is flaming? Flaming is the writing or posting of hostile, offensive, threatening,
insulting, or disrespectful materials online, for example, on web pages and discussion boards, or the
sending of hostile, offensive, threatening, insulting, or disrespectful materials by email, text messages,
or via other means of written or online communication. You are expected to be respectful to all
members of the class and to the instructor at all times. Offensive or inconsiderate emails, text
messages, and/or online or written communication having a hostile or disrespectful tone will NOT be
tolerated. All instances will be reported to the Vice President of Academic Affairs. When you are
VPAA: Revised 9/11, 3/14, 11/16 Page 5
MAT 1033 INTERMEDIATE ALGEBRA
assigned to create examples or to select data or information for an assignment, you are expected to
select topics that are not offensive, threatening, insulting, or disrespectful to any member of the class
or to the instructor. Keep in mind that this is a course for a general audience and, as such, all material
should be appropriate for a general audience. Therefore, you should keep in mind a G-rating (like for
movie ratings, appropriate for general audience) for all materials that you use for assignments, send
to others, or provide for examples during course discussions. Any offensive, threatening, insulting, or
disrespectful materials used in assignments or provided for examples during course discussions, or
sent by email, or via text message will be reported to the Vice President of Academic Affairs and Dean
of Students and the supporting evidence will be forwarded.
ACADEMIC INTEGRITY: According to the Academic Integrity policy, the instructor may: refer the
student to the appropriate support service, assign a reduced grade on the assignment, or assign a
reduced letter grade for the course or give an F as the course grade. The principles of academic
integrity require that all work submitted or presented without citation of sources will be the
students own work, not only on tests, but in themes, papers, homework, and class
presentations, unless it has been clearly specified that the work is a team effort ( p.113 of
College Catalog 2015/2016)
While study groups are encouraged, unless otherwise announced by your instructor, all work turned
in for a grade should be done independently. Any submission by a student of someone elses work as
his or her own or the use of any prohibited aids during testing constitutes cheating. This includes
unauthorized use of a solutions manual, mathematical software or website. The student will earn a
zero for that assignment and will be referred to the Deans Office. A second instance of academic
dishonesty will result in a failing grade for the course. It is each students responsibility to become
familiar with the student code of conduct and academic integrity policy found in the Florida
SouthWestern State College catalog.
ATTENDANCE VERIFICATION
Your attendance in this course will be verified upon completion of the following three assignments:
1. In Canvas:
The quiz titled Syllabus Quiz ;
2. In MyLabsPlus:
The assignment titled Orientation to MMLP;
3. Print the last page of the Syllabus, read it, sign it, and email it to me at cbaker4@fsw.edu
Failure to complete these three activities by January 11 may prevent you from receiving your
financial aid. Please be sure to complete the activities in a timely manner.
The Financial Aid Office must verify that students receiving Title IV funds participate in class.
Students must repay their Title IV funds if not participating through the end of the withdrawal
period. Any student who stops participating in class and does not officially withdraw will be
assigned a grade of F.
The following is the numerical range used for grades in this course:
90 - 100 = A
80 - 89 = B
70 - 79 = C
60 - 69 = D
Below 60 = F
(Note: The incomplete grade [I] will be given only when unusual circumstances warrant. An
incomplete is not a substitute for a D, F, or W. Refer to the policy on incomplete grades.)

Withdrawal Policy:
After two unsuccessful attempts at any course, a student must pay an extra tuition penalty in order to
register in that course for a third time. Therefore, it is important for students to decide immediately
that if they don't wish to remain in a course to withdraw from the course by Wednesday, January 11,
2017. By meeting this deadline, a student will not be recorded as having "attempted" the course, will
not receive any course grade and will be eligible for a full tuition refund.
Students who withdraw after the deadline are not eligible for a tuition refund and will be recorded as
having attempted the course. Students who find, after a time, that they are not able to attend class
meetings should process a Course Withdrawal form as soon as possible. Instructors are not
permitted to initiate these forms; the action must be initiated by a student (or family member in the
case of emergency or illness) in person. Withdrawals cannot be done on-line. The last day to drop a
course without academic penalty is Friday, March 17, 2017.
The college catalog outlines a clear policy for action a student may take in the event of any complaint or
concern that cannot be resolved by student and professor.
Cheating:
Any submission by a student of another students work as his or her own, as well as any prohibited aids
during testing constitutes cheating.
Disclaimer:
This syllabus is an agreement between the instructor and the students registered in the course. From
time to time, this syllabus may need to be amended. The instructor will notify students via classroom
announcement of any changes, additions, and/or deletions to the syllabus. Please print the last page of
this syllabus, sign it, and send me an email with the signed page attached by Thursday, January 12, 2017.
Syllabus Agreement
MAT1033
Professor Baker Spring 2017
1. I understand that there are three requirements to verify my online attendance that must be
completed by Thursday, January 12, 2017. They are:
I must complete the syllabus quiz in Canvas.
I must complete the Mylabsplus orientation in the homework section of
mylabsplus.
I must print out the last page of this syllabus, sign it, and email it to Professor
Baker at cbaker4@fsw.edu
2. I understand that MAT1033 online students are required to purchase a Mylabsplus access code in
order to complete my online homework and quizzes at fsw.mylabsplus.com.
3. I understand that I am expected to complete the homework by the due date of each week. The
semester week starts on a Thursday so they end on a Wednesday and all homework (worth 10%
of my grade) needs to be completed by the Wednesday of a given week. I also understand that any
late assignments will be assessed a 10% penalty.
4. I understand that there are also quizzes due almost every week and once the due date is past, if I
have not taken the quiz, the grade will be a zero and the quiz will not be reopened. Quizzes are
worth 15% of my grade.
5. I understand that there are three chapter tests that I must take and complete by the given test date.
I understand that the first one (worth 15% of my grade) will be taken at home but that the last two
(worth 20% each of my grade) and the final must be taken at a testing site and that I will need to
plan ahead and make arrangements at least a week before taking the test.
6. I understand that I will also need to take a final exam between Thursday, April 27 and Tuesday,
May 2. I understand that the final exam is worth 20% of my grade and that I will receive a 0 if not
taken which will probably result in my not passing the course.
7. I understand that there is help available from my professor via phone calls or emails at
cbaker4@fsw.edu as well as from the math labs located on each campus.
8. I understand that Professor Baker will have online office hours on Tuesday night from 7 until 8
pm through the Canvas Conference tab and if I have questions this would be a good way to get
help.
9. I acknowledge that using notes from my assignments or previous tests (or quizzes), sharing a
calculator, going to websites, using a cell phone, any electronic device, (other than a calculator), or
talking to anyone during a quiz or a test is considered cheating. At the collegiate level, each case
of cheating is handled by the instructor and academic dean. Expulsion from college can result
from such an act.
10. I have read the Syllabus, and I understand the requirements of this course.
