Assignment 2: Newspaper Article (5 marks)

You are required to find an article regards to OSH accidents/issues. The source of the article
should be from magazine or newspaper (August 16 Jan 17). From the article, you are
required to answer the following questions:

1. Identify and explain which type of accident causation theory happens in the
article.
2. Explain TWO (2) effects of the accident.
3. Identify TWO (2) prevention techniques or recommendations that you might
suggest to ensure that the accident will not going to happen again.
SUMMARY OF THE CASE

This case is basically about a worker who is 25-year-old Nepalese national was literally
shredded to death after he was dragged into a box shredding machine after his shoe was
caught on it, where bomba was called to gruesome the accident at a factory in Section 33 in
Shah Alam.

Besides that, the incident took place where the man reportedly used his foot to push the boxes
into the machine but somehow his shoe was caught in the revolving parts of the machine
where this caused his legs to be pulled into the machine up till his waist. Hence, another
worker reportedly switched off the machine when he noticed the man being chewed up and
prevented his whole body from being pulled under.

Furthermore, the victim died at the scene of the accident allegedly due to excessive blood
loss. Whereby, the operation to remove the victim took almost an hour before the body was
handed over to the police for further investigations. Meanwhile, netizens expressed their
horror at the suffering endured by the victim who had died and highlighted how many foreign
workers had met their death trying to earn a living in Malaysia.

Lastly, in nutshell this is tragic where the company must investigate and issue a full
transparent report of what happened and prevent it from recurring and it must say what
compensation the family will receive from insurance and other work benefits, condolences to
the bereaved.
