AFFIDAVIT

I, JOLER DE LA CRUZ JR., Filipino, of legal age, single and with

residence at Taguig City, after having been duly sworn to in accordance with
law, hereby depose and state that:

1. I am a seaman by profession. In my certificate of trainings, the name written

was Joler Dela Cruz Jr., while in my certificate of live birth, the name written
was Joler De La Cruz Jr.. Thus, a discrepancy exits;

2. I hereby declare under oath that the name Joler De La Cruz Jr. and Joler
Dela Cruz Jr. pertains only to me and that the said name corresponds only to
one and the same person;

3. The discrepancy is a mere project of honest human error and typographical

mistake and not intended to create confusion or to commit fraud;

4. Thus, I am executing this affidavit to attest to the truthfulness of the foregoing

to inform the concerned government agencies/instrumentality of the facts
herein and for all legal intents and purposes this may serve.
