REPORT ON MISSING GALAXY TABLETS

BACKGROUND

On Tuesday, 21st July, 2016, the deputy Government statistical (s) set up a committee for the
purpose to investigate into the missing Galaxy Tablets and its accessories from Ghana
statistical service stores.

The committee comprises of the following.

4. Kenneth Yirenkyi Member -Admi. Directorate
5. Timothy Afful Member -Internal Audit
6. Iddrisu Andani Member -Chair Local Union
7. Mbgamsi Gabriel Member -Security Coord

The committee started its work on 26th July 2016 and the investigation lasted for a period
of 14 days. The following officers were all invited to testify about what they know about the
missing tablets.

The store is made up of three rooms. The big room is used as a ware house, whilst
the middle room is used as an office and the third one is used as a mini ware
house, attached to the office in which the missing items were kept. The three
officers have their respective keys to all the rooms. The head of stores,
Mohammed Abdullai has his office detached from stores whilst Mariam Ibrahim
and Eric Agyemang occupy the same office which is semidetached to the
warehouse.
MARIAM IBRAHIM
On the 24th of June, 2015. Mariam Ibrahim took delivery of the following items to
stores.
1. Total number of Samsung Galaxy Tab A = 60
2. Total number of Micro SD 32 GB = 60
3. Total number of Micro SD 64 GB = 60
4. Total number of cover case = 60

On the 11th November, 2015, she issued out seventeen of the tablets to IT
section upon a memo from IT Director. On another occasion, she instructed a
formal national service person by named Famous Agbozo who completed his
national service the previous year at the stores, to make some entries into the
ledger allocation book. It was also revealed that Famous Agbozo used to visit
the stores and assist the officers in their daily operations.
It was during such operations that Famous Agbozo detected the anomalies in
the ledger book concerning the total number of tablets and reported the issue
to Eric Agyemang the next day whilst Mariam had left for Wa.

FAMOUS AGBOZO
He stated that, he happened to do his national service at GSS stores
department whereby he had the opportunity to come around and help, even
after his service was over. On one occasion, he visited the office when Mariam
asked him to make entries into the ledger book. It was during this exercise that
he found out that, the entries had already been made issuing out the whole 60
Galaxy tablets whilst some were still in the stores. To this effect, he drew the
Agyemangs attention.

ERIC AGYEMANG
On his part of the story, he stated that, on the 12th November 2015, he was
having a training programme at the GSS training centre together with Abdullai
Mohamed. He was instructed by Abdullai Mohamed to issue out 11 Galaxy
tablets to IT personnel led by Nana Debrah for official use. He obliged and
brought all the tablets into the training room; whereby the number required
were picked by the IT personnel and the remaining 33 was sent back to stores.
Agyemang on his, confirmed the report by Famous Agbozo about the
anomalies he detected in the ledger book and quickly called Mohammed
Abdullai on his cell phone and informed him about Famous observation and
also the disappearance of the two tablets. Mohammed and Agyemang later
went to the stores and did recounting after which, both confirmed that the two
tablets could not be found.

EMMANUEL NANA DEBRAH


Who on two occasions lead the IT personnel in the collection of the galaxy
tablets testified that, soon after the receipt of the second round of the 11
tablets, the remaining 33 were checked and handed over to Agyemang who
then took custody for safe keeping at the stores. His evidence was supported
by one national service person by name Solomon Kekeli from IT department.

MOHAMMED ABDULAI
The head of stores stated that, on the 24 of June, 2015, 60 galaxy tablets were
bought by the office for official use. On the 11th of November 2015, Director
of IT issued out a memo with distribution list attached for the issue of
seventeen (17) of the tablets. He made an entry to the ledger book as having
released all the 60 tablets to director of IT through Nana Debrah. Sixteen (16)
were selected and remaining 44 tablets were brought back to stores. The next
day, which is 12th of November, 2015, IT Director again instructed for the
release of 11 tablets which Mohammed Abdullai instructed Agyemang to go
ahead and issue the order. Agyemang confirmed the instruction and obliged
accordingly.
Agyemang and Solomon kekeli did the final checks of 33 tablets and sent the
remaining 33 back to stores.

FINDINGS

1. After all the investigations, it was clear that no part of the stores was forced
opened or broken into.
2. It was clear that, on the 11 and 12th November, 2015, 27 of the 60 galaxy tablets
were officially issued out and the remaining 33 were sent back to stores.
3. No proper entries were done into the stores allocation ledger book.
4. Famous Agbozos frequent visit and workings at the stores after his service was
wrong.
5. Mariam Ibrahim was wrong to have engaged Famous Agbozo who had no
business at that point in time to do the entries in the ledger book and also left
alone in the stores without any supervision.
6. Mariam was the person who requested Famous Agbozo to make entries into the
ledger book, why did he not report back to Mariam his observation about the
anomalies but rather chose to report to Agyemang.
7. From our visit to stores, it was clear that there was no security locks at the mini
store room where the tablets were kept.
8. There was sense of negligence of duty on the part of the three officers who work
at the stores.
