& SERVICE CONTRACT KNOW ALL MEN BY THESE PRESENTS: This contract executed and entered into this Philippines, by and between: PRIVATIZATION AND MANAGEMENT OFFICE, an agency of the Government of the Republic of the Philippines established under Executive Order No. 323 dated 06 December 2000, with office address at 104 Gamboa Street, Legaspi Village, Makati City, represented herein by its Chief Privatization Officer, GUILLERMO N. HERNANDEZ (hereinafter referred to as “PMO”); and POWER HOUSE PEST CONTROL SERVICES, a company organized and existing under Philippine Laws with business address at 2422 R. Fernandez St., Gagalangin, Tondo, Metro Manila herein represented by ANNA MARIE M. ESCOBER (hereinafter referred to as the “CONTRACTOR”. WITNESSETH: that WHEREAS, PMO requires support services, particularly Pest Maintenance Services to facilitate the performance of its mandated function; day of __ Manila, Control WHEREAS, the PMO conducted a public bidding on April 29, 2009 for Pest Control Maintenance Services in accordance with the provisions of Republic Act No. WHEREAS, the CONTRACTOR is a duly licensed business entity engaged in the business of providing support services, particularly for Pest Control Maintenance Services staffed with trained personnel, fully equipped with tools, materials and ‘equipment used in the performance of said services; WHEREAS, after thorough evaluation and verification of the bid documents, the IO Bids and Awards Committee (“PMO-BAC”) determined that the CONTRACTOR ae (‘RA 9184”) and its Implementing Rules and Regulations, as amended; So. submitted the lowest calculated and responsive bid for Pest Control Maintenance Services; WHEREAS, upon recommendation of the PMO-BAC, the Chief Privatization Officer approved the award of the Contract for Pest Control Maintenance Services to the CONTRACTOR; NOW, THEREFORE, for and in consideration of the foregoing premises and mutual covenants hereafter set forth, the parties have agreed and bound themsel¥es as follows: 1. The CONTRACTOR shall provide pest control maintenance services at the ground up to the sixth floor of the PMO building located at 104 Gamboa Street, AWSERVICE CONTRACT By and between PMO and Power House Pest Control Services Legaspi Village, Makati City, including its basement and parking lot, and at the SCAIC Warehouse in Bagbaguin, Valenzuela City. (“Premises”) 2. Pest Control Maintenance Services to be rendered by the CONTRACTOR, shall involve the following: 2.1. COCKROACH INFESTATION ii, INSPECTION The CONTRACTOR shall inspect indoor areas of the Premises for cockroach infestation including but not limited to harborages such as cupboards, floor drains, cracks and crevices. The CONTRACTOR shall also inspect outdoor areas of the Premises including but not limited to the drainage system to determine the presence of cockroaches as well as recognize the possibility of re- infestations from adjacent premises or from vegetation. Monitor cockroach infestation within the premises by gathering information thru office complaints in addition to the observation of the CONTRACTOR'S service technician. b. RESIDUAL INSECTICIDAL SPRAYING i, The CONTRACTOR shall intensify residual spraying to areas of the Premises where insects congregate, crawl or hide particularly cracks and crevices. ii, Pyrethroid chemical shall be used to flush the cockroaches out of infested areas at the Premises and to determine the exact location of infestation. iii, Crevices and cracks near food preparation centers and equipment shall be sprayed regularly by the CONTRACTOR. « _ INSECTICIDAL BAIT APPLICATION i, Whenever necessary, the CONTRACTOR shall apply cockroach baits near harborages and other infested areas, such as corners of electrical wiring, telephone apparatus, ete. 4. FREQUENCY OF TREATMENT ‘Treatment shall be done twice (2x) a month for PMO Building, 2.2, RODENT INFESTATION INSPECTION ‘The CONTRACTOR shall conduct regular inspection of all potential harborages and food sources of rodents and all rodent jv[SERVICE CONTRACT ‘By and bewoen PMO and Power Howse Pest Control Servicer : poison baits, traps, bait stations and other devices installed at the Premises. b. BAITING ‘The CONTRACTOR shall install bait stations in locations where rodents are likely to be found in the Premises. The CONTRACTOR shall replace or install additional baits when needed. © TRAPPING ‘The CONTRACTOR shall install cage traps, glue boards, snap and other chemical devices to supplement and strengthen the baiting technique. 4. FREQUENCY OF TREATMENT ‘Treatment shall be done twice (2x) a month for PMO Building and quarterly for SCAIC Warehouse. 2.3. FLYING INSECT CONTROL a. INSPECTION ‘The CONTRACTOR shall conduct a thorough inspection of the Premises to determine the degree of infestation, entry points and unforeseen breeding sites of mosquitoes and other flying insects. b. FOGGING The CONTRACTOR shall use a fogging machine in the Premises, including but not limited to the drainage systems to spray chemicals that control the infestation of mosquitoes, flies and other flying insects. © MISTING The CONTRACTOR shall use a misting machine used inside rooms, the kitchen and food preparation areas that disperses minute droplets of insecticide solution to penetrate of hard to reach or inaccessible areas. d. FREQUENCY OF TREATMENT. Treatment shall be done twice (2x) a month for PMO Building. 3, TERMITES INFESTATION WW The CONTRACTOR shall also provide trophallaxis treatment to the building and records area in all floors of the PMO building and SCAIC Warehouse in Valenzuela City.SERVICE CONTRACT By ond berweon PMO and Power House Pest Control Series ‘The treatment of building shall be done once, immediately at the start of the contract period and once every quarter for all records area in all floors of PMO building and SCAIC Warehouse in Valenzuela City. 4. In consideration of the above Pest Control Maintenance Services, the PMO shall pay the CONTRACTOR the sum of NINE THOUSAND FIVE HUNDRED EIGHTY THREE and 33/100 ONLY (2 9,583.33) per month one (1) week after the completion of the last treatment per month, provided however the CONTRACTOR shall have delivered to the PMO its service report and invoice. 5. This service contract shall be for a period of one (1) year starting June 1, 2009 up to May 31, 2010 unless sooner terminated by the client thru written notice to the CONTRACTOR at least THIRTY (30) DAYS prior to the intended termination date. 6 The CONTRACTOR shall give PMO a SERVICE WARRANTY, which will require the CONTRACTOR to provide treatment anew to areas at Premises where reinfestation has occurred, without additional cost to the PMO. ‘The SERVICE WARRANTY shall not be precedent to a twice a month services which also has its corresponding amount. 7. The CONTRACTOR binds itself to utilize only government approved chemicals in the rendition of its services to the PMO and warrants that such chemicals will not cause any harm whether immediate or future, direct or indirect, to any person, nor will they cause any damage to the environment.
