SUBJECT: San Luis Obispo County Air Pollution Control District Notice of Violation Number 2943

Dear Mr. Sidun:

Enclosed please find San Luis Obispo CountyAir Pollution Control District (District) Notice of Violation
Number 2943, issued for the operation of a crude oil tanker truck receiving rack at the phillips 66, Santa
Maria Refinery located at 2555 Willow Road in Arroyo Grande beginning on February 10,2016. This ii a
violation of San Luis Obispo County Air Pollution Control Distrlct permit to Operate Number 44-52,
condition lll.A.1.l, condition lll.A.1.m, condition lll.C.6.a, 40CFR60.7_and District Rule 206, Conditional
Approval.
Please advise the Air Pollution Control officer in writing on or before May 24,2016 of corrective action
taken and measures taken to prevent further violations of this type. Upon receipt of your response the
District will contact you regarding settlement of this violation.
District Rule 202, Permits (copy enclosed), requires any person who uses equipment which may cause the
issuancq of air contaminants or eliminates, reduces, or controls the issuance of air contaminants to first
obtain a Permit to operate from the Air pollution control officer (Apco).
San Luis Obispo County Air Pollution Control District Permit to Operate Number 44-52, condition 1t.A.1.m
requires the APCo to be notified in writing before any changes are made in the design, construction, or
method of operation of this equipment, or any modifications are made to process conditions that might
increase the emission of air contaminants in excess of existing permit limits, for those emission
unit and
pollutant combinations with such limits, or that might increase the potential to emit
of any air contaminant,
for those emission unit and pollutant combinations without current limits.
Therefore, also enclosed, please find san Luis obispo county Air Pollution
control District (District) Notice
to Apply Number 0482 and a Permit Application Form for a crude oil tanker
truck receiving rack. please
complete the application form and return it to the District along with
the $1g5.00 filing fee by May 24,2016.
