Contractors are cautioned to read each question carefully. Some of the questions may contain several
parts and therefore, require several responses. The questions must be answered in the same order as
asked, using the enclosed forms where applicable. Failure to provide any of the requested information,
or providing the information in a format other than directed, may result in your Qualification
Questionnaire not being evaluated. Please be advised that all information provided herein is subject to
verification. Also, contractors are advised to ensure that all information submitted is accurate and does
not include any misrepresentation of contractor qualifications. Such situation can have a serious
negative impact on contractors chances of being considered for future SAUDI ARAMCO work in
general.
Qualification Questionnaire
For
Non-GBS Specialized Scaffolding Contractors
Note: Per GI 8.001, para. 3.3, only Specialized Scaffolding Contractors are permitted to design and
erect special scaffolds and scaffolds over 12.2 meters (40 ft.) tall. Specialized Scaffolding
Contractors may be on the SAUDI ARAMCO General Bid Slate (GBS) for Scaffolding Services if they
offer scaffolding services to others (GBS Specialized Scaffolding Contractors), or not on the GBS if
they do not offer scaffolding services to others (Non-GBS Specialized Scaffolding Contractors). This
questionnaire must be completed by those SAUDI ARAMCO contractors who: (1) are seeking to
qualify as a Non-GBS Specialized Scaffolding Contractor for the purposes of GI 8.001, (2) do not want
to offer scaffolding services to others, and (3) have a separate scaffolding division/unit. Qualified Non-
GBS Specialized Scaffolding Contractors (not on the General Bid Slate) are permitted to design and
erect special scaffolds and scaffolds over 12.2 meters (40 ft.) tall for their own use only, using their
own personnel and materials.
On Exhibit Form A1, provide at least TWO (2) scaffold plans, including drawings and
structural calculations, for EACH of the following 5 types of scaffolds (submit at least 10
scaffold plans):
1. Birdcage Scaffolds over 12.2 meters (40 feet) tall
2. Independent Run Scaffolds over 12.2 meters (40 feet) tall
3. Underhung Scaffolds over 30 m2 (320 ft2) total platform area
4. Scaffolds over 38 meters (125 feet) tall for a slender vessel or stack
5. Other Types of Special Scaffolds (see GI 8.001 for definition of Special Scaffold).
All 10 scaffold plans must be either actual scaffold plans from your past contracts with an
oil/gas, chemical, or other industrial company (within Saudi Arabia), or your own unique sample
scaffold plans that have been locally prepared specifically for this evaluation. All (actual or
sample) scaffold plans shall demonstrate your companys current design and engineering
capabilities (have been prepared by currently available in-house and/or contracted personnel that
your company will use for future jobs) and shall have been completed within the past 12
months. All drawings and structural calculations shall be prepared (or reviewed and
approved/signed) by structural engineer(s) employed by a Saudi Aramco approved General
Engineering Services (GES) consultant.
Scaffold plans shall be complete (submit signed SA Form 9613 Scaffold Plan Completeness
Checklist with each plan) and include structural calculations and drawings (on at least size
C paper - 14 x 20 inches).
Submit two (2) paper hardcopies of each scaffold plan (drawings and calculations) and all other
required documentation (Exhibits). Each scaffold plan and Exhibit shall also be submitted as a
PDF file on a CD-ROM. All scaffold plans and Exhibits shall be clearly organized and
labeled.
Scaffold plans shall be in full accordance with the requirements of the latest revision of the
SAUDI ARAMCO Scaffold Safety Handbook (SSH), including the latest version of
Addendum #2.
On Exhibit Form A2, provide contractual information for your companys last four (4)
completed contracts that included Applicable Contract Work, within the past 5 years,
preferably with an oil/gas, chemical, or other industrial company.
The Applicable Contract Work must be described in detail, including scope and monetary
value of Applicable Contract Work, contract date, size and type of scaffolds erected, design
criteria, quantity and type of materials used, inspection and tagging requirements, etc. These
contracts, awarded to your company as a primary contractor or subcontractor, shall include
Applicable Contract Work as part of the work scope.

Qualification Questionnaire Non-GBS Specialized Scaffolding Contractor
The following types of previous contracts may be submitted, in decreasing order of preference:
1. Contracts with SAUDI ARAMCO.
2. Contracts inside the Kingdom of Saudi Arabia, but for oil/gas, chemical or other
industrial companies other than SAUDI ARAMCO.
3. Contracts for oil/gas, chemical or other industrial companies outside the Kingdom of
Saudi Arabia.
4. Other types of contracts.
Contractors shall complete all the applicable information requested under Exhibit Form "A2".
Use a separate sheet for each contract (4 min.).
Provide evidence that your Organization currently and directly employs an adequate number of
Scaffold Supervisors I and Inspectors that are certified by SAUDI ARAMCO in accordance with
GI 8.001 (see minimum numbers stated above). Submit photocopies of the front and back of
their SAUDI ARAMCO Scaffold Supervisor I or Inspector certification ID cards.
In addition to submittal of resumes, the PET may interview each of your Organizations
Structural Engineers (2 min.) who are available for work on SAUDI ARAMCO contracts, to
verify whether they have sufficient knowledge and experience for the design of Special Scaffolds
(as defined in GI 8.001). Please ensure current telephone numbers are provided so interviews can
be arranged.
Complete Exhibit Form C. While not preferred, photographs or other documentation may be
submitted (preferably on CD-ROM), in lieu of actual samples of your scaffolding materials, whenever
requested. Also, provide written proof of the quantities and material specifications of all scaffold
materials available for use within SAUDI ARAMCO that are currently in your companys inventory
(i.e., copies of actual Purchase Orders, Invoices, Bill of Lading, etc).
3.1 Submit at least five (5) recent (within the last 2 years) scaffold tubing test reports (that include
yield strength, tensile strength, wall thickness, etc.) from BOTH the pipe manufacturer and an
independent mechanical testing laboratory for tube and coupler scaffold tubing and for each type
of system scaffold tubing to be used within SAUDI ARAMCO. Refer to SAUDI ARAMCO
Scaffold Safety Handbook (paragraph 9.7.2) for material and testing requirements.
3.2 Submit at least two (2) 1-foot long samples of embossed scaffold tubing from the inventory of
scaffold materials to be used. Refer to SAUDI ARAMCO Scaffold Safety Handbook (paragraph
9.7.2) for embossing requirements.
3.3 Submit at least two (2) sample right-angle (load bearing) scaffold couplers and at least two (2)
recent coupler test reports from an independent mechanical testing party showing the couplers
Safe Working Load (SWL) and that it meets coupler specifications and materials. Refer to
SAUDI ARAMCO Scaffold Safety Handbook (esp. paragraph 9.7.2) for coupler requirements -
including the specified markings on each coupler.
3.4 Submit one (1) sample of each type of miscellaneous scaffold accessories, such as ladder &
toeboard clamps, screwjacks, etc., to be used within SAUDI ARAMCO.
3.5 Submit one (1) sample of each system scaffolding component (standard, ledger, transom,
intermediate transom, etc.) to be used within SAUDI ARAMCO.
3.6 Submit at least two (2) copies of manufacturers technical data, test reports, and one (1) 1-foot
long sample of each type of scaffold platform unit (solid sawn wood plank, laminated veneer
lumber plank, fabricated plank, etc.) to be used within SAUDI ARAMCO. Refer to SAUDI
ARAMCO Scaffold Safety Handbook (esp. Paragraph 9.7.3) for platform unit requirements -
including the specified visible markings on planks.

3.7 Submit at least two (2) copies of manufacturers technical data and one (1) product sample of
each of the personal fall arrest system (PFAS) components (full-body harness, shock-absorbing
lanyard, self-retracting lifeline, etc.) to be used within SAUDI ARAMCO. Refer to SAUDI
ARAMCO Scaffold Safety Handbook and Construction Safety Manual for PFAS requirements
(including being labeled as meeting ANSI Z359.1 or equivalent standards).
Note: Exhibit Form D has been deleted from this technical evaluation since a separate
pass/fail safety evaluation is now performed as per the Saudi Aramco Safety Management Guide
for Contractor Pre-Qualification Safety Evaluation.
5.1 Submit evidence and documentation of your organizations established scaffold training program
for scaffold supervisors, inspectors, and craftsmen (scaffolders), including numbers of
personnel who have recently completed this training. This training program can include use of
recognized outside training agencies.
5.2 Submit evidence (photocopies of scaffold tags, tagging procedure, etc.) that your organization
has past experiences using a Scaffold Tagging System (preferably SAUDI ARAMCOs).
5.3 Submit at least one (1) actual Hazard Identification Plan (HIP) for scaffold erection, alteration,
and dismantling (including PPE and hand tools to be used).
5.4 Submit written guidelines for documenting and communicating to SAUDI ARAMCO
representative any temporary manpower, transportation, or material shortages that could delay
the work.
5.5 Submit written plans for rectifying any temporary manpower, transportation, or material
shortages that are highlighted by SAUDI ARAMCO representative.
5.6 Submit evidence (i.e., copies of historical records of quality checks) that your organization
regularly conducts Internal Quality Assessment (QA) of scaffolding materials and has actually
repaired or removed deficient materials from service.
