Instructions: After every audition, you should take a moment to complete an audition
log sheet to help you recall the events of your audition when preparing for a callback. It
will also serve as a record of auditions completed and reflect your growth.

Who was in the room? Who did you audition for (the names, if possible)?

What song(s) did you present?

Dress: What did you wear? Was it appropriate?

How did it go? (Note comments they made, how you felt, any information you received.)

Were you asked to do a reading from the script?

Did you get a callback: Yes or no?

If so, where/when is it?

Did they tell you what to preparefor the callback?

Did you send a thank-you or follow-up note/letter?

Were you given any info on the schedule (rehearsal dates, show schedule)?

Contacts: Did you meet anyone at the audition? Did you find out about anything you
want to follow up on?
