Lissette was notified about the meeting by email to both her personal and

Age Appropriate Transition Assessments and Summary
Personal Interests:
Lissette has expressed interest in the food service industry as well as
cleaning and laundry. She has also expressed interest in getting married and
having children.
Training/Education:
Lissette will complete her high school years in May 2017. She has completed
many training opportunities throughout her high school career.
Employment:
Lissette has had unpaid work training opportunities in the community
through her high school transition services program.
Independent Living Skills (Optional):
Lissette will have a written plan that outlines the cost of transportation,
utility bills and rent for a house or apartment.

Who provided information about student strengths, preferences, interests:
Lissette provided the information about her preferences and interests. Her
parents provided the information about her strengths as well as Jennifer
Lesch who is her special education teacher.
Was invitation of an outside agency considered? Were they invited? Did they
attend?
Jodi Lane is an academic advisor from Montgomery County Community
College was invited and was able to attend the meeting.
Measurable Post-Secondary Goals
Training/Education Goal:
After graduation Lissette will attend courses at Montgomery County
Community College to gain skills in functional academics and career
preparation.
Training/Education Connected IEP Annual Goal:
Lissette will complete her application to Montgomery County Community
College independently and show it to her special education teacher by May
2017.
Employment Goal:
After graduation Lissette will apply for to do a job shadow in either the food
service industry or the cleaning and laundry industry.
Employment Connected IEP Annual Goal: Lissette will work part time in
either the food service industry or the cleaning and laundry industry
while attending college.
Independent Living Goal (When necessary):
Lissette will have a plan that outlines the cost of transportation, utility bills
and renting a house or apartment.
Independent Living Connected IEP Annual Goal:
Lissette will call 2 different apartment complexes and 2 different housing
rental companies to compare the costs as well as inquiring about
transportation costs.
