PEER FEEDBACK RE: CLINICAL PERFORMANCE

NAME OF STUDENT RECEIVING PEER FEEDBACK: Mikkel-Brandon De Leon

I think you are doing a great job at:

1. MEDICATION ADMINISTRATION:

Before giving medications to his clients, Mikkel does his research before the beginning of the

shift to make sure he has the knowledge about the medications he will give. Mikkel also prepares

his medications ahead of time and follows the 6 rights of medication administration.

2. DEVELOPING A THERAPEUTIC RELATIONSHIP WITH CLIENTS:

Mikkel uses therapeutic communication to his clients by getting to know them, asking how they

are, helping them according to their needs, and by advocating his clients to meet their needs.

3. DOCUMENTATION:
Mikkel completes his iView charting on time with relevant information according to his head-to-

toe assessments and vital signs taken from his clients. Mikkel also asks questions to his peers, CI,

and staff whenever he has problems documenting on the Powerchart system.

I think you could develop further in the following areas:

1. HEAD-TO-TOE ASSESSMENT:
Although Mikkel is doing great in his assessments, he is encouraged to practice more in order to

get accurate data. By practising more, it can help him perform his assessments in an organized

manner.
2. PROCEDURES/SKILLS
Mikkel demonstrates good knowledge to the procedures that he will perform (eg. giving enteral

feeds). However, he is encouraged to read back his checklists from lab and to utilize his lab kits at

home to get familiarize with the steps.

3. ORGANIZATION
Mikkel completes his tasks on time, but he needs to use an organizational sheet that can help him

manage his tasks and guide him throughout the shift.
