Important Instruction

We would like you to keep this diary of everything you eat and drink over the
next two (2) days (1 weekday day and 1 weekend day*).
It is important that you do not adjust what you eat and drink just because you
are keeping a record. Please continue to eat what ever you wish.
(* weekend for Kedah, Kelantan dan Terengganu : Friday and Saturday; Selangor,
Johor,Perak, Pahang, Sabah,Penang : Saturday and Sunday)

General Instruction
1. Please eat at your habitual consumption.
2. Record everything you eat and drink including snack .
3. Complete the Meal and Place Prepared columns for each meal or snack.
Please write in name of restaurant if you know it.
4. Please record all the food taken right after each meal, NOT based on one
day recall.
5. It is necessary to jot down the brand of instant or processed food.
6. Please give a clear and brief description regarding on the measurement
used.
Example: 1 teaspoon level of oil or 1 dessert spoon heap of sugar

7. Describe briefly and accordingly all the food taken for each meal.
Example: Chicken - fried or roasted
- how many pieces?
- with/out skin
Butter - how many tea spoons?
- purpose (bread spread, )
- brand

8. Start each new day on a new page.


9. Please write CLEARLY and BRIEFLY with pen.

How to Record Each Food

Describe each food and beverage in detail, as best you can.

1.Placed prepared: Where it is prepared ; home or restaurant/stall?

3.Food & beverage: Record food and beverage for each meal (a) breakfast (b)
Morning tea, (c) lunch, (d) tea ,(e) dinner or (f) supper
Type of food and beverage taken. Please record type of food
and beverage taken according to space given(please refer to
the picture as indication).
Give a brief explaination on the content s for composite dish.
Example : Nasi lemak
1 cup of nasi lemak
2 dessert spoons of sambal tumis
2 slices of cucumber
boiled egg
1 dessert spoon of fried anchovies.
Please state the brand for the processed food.
Example : Sardin ( King or Ayam Brand ), instant noodle (
Maggi or Mamee)

4. Description & Explain the method of food preparation (fried,roasted,


preparation: steamed,boiled). Please state type of oil or fat used for cooking.
5. Portion size: For food portion or size estimation, use the following as guide:
i. Refer to example given (page 7)
ii. Refer to guideline to record description and
preparation and portion size (page 5) and
iii. Refer mini food album

Rice Brown or white; boiled or fried Cup, scoop
Fruits (fresh) -what sort and variety, cored; with or Slices, whole
without skin.

Vegetables Type or variety, fresh or frozen; how Dessert spoon, cup


cooked: fried, boiled, steamed
Biscuit Plain cracker, cheese, Piece
sweet/sugared, wafer (state the
brand if possible); home made
biscuits (state the ingredients).
Fish What sort; fried, boiled, grilled, Food album, weight
salted, breadcrumbs; tin with oil or on the label
tomato sauce, give brand.
Meat What sort : lean or fatty, fried, Slices
roasted, with or without gravy.
Bread/bun Wholemeal, white bread; current or Slices
fruits. State the brand.

Butter, honey, jam Type of spread; butter : ordinary or Dessert spoon or


low fat diary spread; jam and honey teaspoon
:specify if low sugar. write in the
brand
Margarine State the brand Dessert spoon or
teaspoon
Breakfast cereal, barn, What sort: cornflakes, weetabix, Dessert spoon,
wheat germ muesli etc; give brand if possible. bowl
Coffee or tea with/ without sugar or milk (state the Cup or glass
amount of sugar, type and amount of
milk), instant (write in the name of
brand if possible, weight on the

Milk- for drinking on its full cream, skimmed, UHT, flavoured Cup or glass
