Important Instruction
We would like you to keep this diary of everything you eat and drink over the
next two (2) days (1 weekday day and 1 weekend day*).
It is important that you do not adjust what you eat and drink just because you
are keeping a record. Please continue to eat what ever you wish.
(* weekend for Kedah, Kelantan dan Terengganu : Friday and Saturday; Selangor,
Johor,Perak, Pahang, Sabah,Penang : Saturday and Sunday)
General Instruction
1. Please eat at your habitual consumption.
2. Record everything you eat and drink including snack .
3. Complete the Meal and Place Prepared columns for each meal or snack.
Please write in name of restaurant if you know it.
4. Please record all the food taken right after each meal, NOT based on one
day recall.
5. It is necessary to jot down the brand of instant or processed food.
6. Please give a clear and brief description regarding on the measurement
used.
Example: 1 teaspoon level of oil or 1 dessert spoon heap of sugar
7. Describe briefly and accordingly all the food taken for each meal.
Example: Chicken - fried or roasted

How to Record Each Food
3.Food & beverage: Record food and beverage for each meal (a) breakfast (b)
Morning tea, (c) lunch, (d) tea ,(e) dinner or (f) supper
Type of food and beverage taken. Please record type of food
and beverage taken according to space given(please refer to
the picture as indication).
Give a brief explaination on the content s for composite dish.
Example : Nasi lemak
1 cup of nasi lemak
2 dessert spoons of sambal tumis
2 slices of cucumber
boiled egg
1 dessert spoon of fried anchovies.
Please state the brand for the processed food.
Example : Sardin ( King or Ayam Brand ), instant noodle (
Maggi or Mamee)
