HEALTH BY DESIGN AT A GLANCE

WHAT IS HEALTH BY DESIGN (HBD) AND HOW DOES IT WORK?


HBD is a voluntary health incentive program for Jacobs employees and their spouses or domestic
partners who are enrolled in a Jacobs medical plan with UnitedHealthcare (UHC).

HBD enables you to reduce both health risks and health care costs. By participating in HBD, you can
lower your 2017 medical premiums by completing various health actions (see What Incentives Can You
Earn? below).

WHAT INCENTIVES CAN YOU EARN?


You and your spouse or domestic partner can earn incentives by choosing from any of the following
activities.

Health Awareness Complete the Rally Health Survey $125
Complete a biometric screening $125
Meet or exceed the health targets for the
following:
Body mass index (BMI) - Test 30.0 or $125
Healthy Outcomes
decrease BMI by 1.0 from prior year
Blood pressure - Test 140/90 $125
Total cholesterol - Test 240 $ 75
Glucose - Test 140 $ 75
Complete the following healthy activities:
3 Rally missions $ 50
Healthy Activities
1 Rally walking challenge $ 50
An EAP stress management course $ 50
A financial planning course $ 50
TOTAL Up to $650 per person
Attest that you have been tobacco-free for the
Tobacco-Free $150 per person
past 12 months.
TOTAL MAXIMUM Up to $800 per person
ANNUAL INCENTIVE ($1,600 family maximum)

ENRICH YOUR WELLNESS JOURNEY WITH RALLY!


Beginning Sept. 1, you will have access to Rally (available at myuhc.com > login/register > Rally Learn
More), a personalized, interactive health and wellness platform from UHC that helps you take an active
role in your health. More information on making the most of Rallys online tools and resources, including
special UHC discounts on exercise devices, will be forthcoming.

WHAT IS THE DEADLINE FOR COMPLETING AND SUBMITTING HEALTH
ACTIONS?
Dates to complete health actions are:

For existing UHC participants: Dec. 31, 2016.


Existing participants who complete and submit their health actions by Oct. 31, 2016 will have their
HBD incentive (medical premium reduction credits) applied beginning with the first paycheck in
2017. Those who complete and submit their health actions between Nov. 1 and Dec. 31, 2016 will
have their health incentive credit applied to their first paycheck in Mar. 2017 (retroactive to Jan. 1,
2017).

For new UHC participants: The later of Dec. 31, 2016 or 90 days from UHC coverage start date.
Premium incentive credits begin approximately 60 days following the completion and submission of
health actions to UHC (incentives are retroactive to the UHC coverage start date).

For all UHC participants: Premium incentive credits are prorated and applied to each
paycheck provided you and your spouse or domestic partner continues to be enrolled in a
Jacobs UHC medical plan.

Important Note
You and/or your spouse or domestic partner may still be eligible for HBD incentives if unable to complete the
biometric screening tests, achieve the desired health targets and/or complete a wellness coaching program due to
a medical condition. If it is unreasonably difficult or medically inadvisable for you to participate in the HBD tests or
wellness coaching programs and/or to achieve the desired health targets, your doctor can request a medical
alternative on your behalf by completing the appropriate section on the Provider Notification Form and submitting
to UHC.

The purpose of Health by Design is to encourage behaviors to maintain and improve your health which, in turn,
helps control health care costs for you and the Company. While Jacobs expects the program to continue, we
reserve the right to modify or terminate the program and any of its features, including the maximum annual
incentive, at any time and for any reason.
