MEMORANDUM OF AGREEMENT
This Memorandum of Agreement [MOA] is made and entered into this _________ day of

/guardian of our student(s) in the DMC College Foundation, Basic Ed. Department.

Ms._______________________________; High School Principal of the DMC-SHS, DMC

College Foundation Inc.

This Memorandum of Agreement is to outline the terms of implementation, responsibilities and

conditions on the matter pertinent to the case(s) involvement of the student for the probationary
period in the start of the school year.

It is being understood and agreed upon during the parents conference with the Disciplinary
committee,
1. THAT the student(s) committed and repeated the violations in the School policy resulting to
this MOA;
2. THAT in the event of any violation to the School Policy will result to Dismissal and non-
admission of the student in the DMC College Foundation, and advice to transfer will be
implemented;

3. THAT the parents/ guardian will constantly remind their student to follow the agreement
for the period of one (1) year, subject for a renewal of this agreement when they wish to
continue studying in this institution.
4. THAT this agreement is entered into with mutuality of parties and the witnesses thereof.
5 THAT this Memorandum of Agreement consist of one (1) page and shall be effective upon
the completion of the signatories below.
