MONTHLY SUMMARY
MANAGERS SUMMARY
(Include significant events, changes, Accidents/Incidents & follow-up, VA Reports & outcome)
XXX had a good month with Legacies. During January, he completed his chore program, most
days, without multiple prompts from staff. He has shown wonderful improvement in is respectful
communication, to staff, this month. XXX does a good job at communicating with is staff when he
does not want to talk or needs time to process through his thoughts. On a few occasions, in
January, XXX had a hard time getting his emotions back to a good baseline and being respectful
in that process. Staff step in and help redirect XXXs behaviors in a more positive way. During
January, XXXs staff had informal discussions with him about personal boundaries. XXX seemed
interested in getting to know some of his staff more and asked them appropriate questions. On
one occasion, XXX stated to a staff that when he gets older he would like to attend Winona State
University. During the end of January, XXX showed more aggressive behaviors. On several
occasions, he would punch himself in the head or on the body, hit his head against the wall and
call himself dumb. During one of his rough shifts, XXX was punching himself in the head and mom
felt that it was best to put him in a hold, which she administered. Though XXX had a rough end of
the month, he did fantastic in other areas. During January, XXX needed just a few reminders to be
respectful to peers but, enjoyed going to the Rec to hand out and play with other peers. He also
utilized the Legacies building, to play board games, socialize and make crafts with staff.
