SEMS Mid-Year Parent/Community Survey

The partnership between home and school is a strong indicator of scholars success. We appreciate your participation in
providing feedback that can be used as we strive to continuously improve this partnership. Once you have completed the
survey, please give it to your scholar and have them return to his/her homeroom teacher. If you have more than one
scholar, please complete 1 survey per scholar. We are requesting all surveys be returned by Friday, April 7th. A reward
will be given to the grade level with the most responses, as well the individual homeroom with the most.
