STUDENT RESPONSE AND ASSESSMENT TOOLS

Name: Amber VanRoyen

Grade Level: 5th grade
Content Area: Social Studies
Standards Addressed: For this activity, I decided to create a pre-test that will assess my students
knowledge about social studies, more specifically history. Therefore, I include many questions that
correspond to different standards. Some of these standards include: SS5H6 Describe the importance of
key people, events, and developments between 1950-1975. SS5H4 Explain Americas involvement in
World War II. SS4H5 Explain the causes, major events, and consequences of the Civil War
Student Response Technology Used:
NearPod Socrative PollEverywhere iRespond TodaysMeet Other:
Technology that students will use to respond to questions/prompts:
Computer Hand-held student response system (such as iRespond) Phone
Tablet (such as iPad Other wireless device (such as iPod Touch)
Describe the instructional activities that will occur prior to the SRS activity and how you will
introduce the SRS activity: Due to my SRS activity being a pre-test there will be no prior instructional
activities but I will introduce this as a test that isnt for a grade, it is just to help know how each student is
doing in social studies and what they know or dont know.
Describe the purpose of the SRS activity (check all that apply):
Assess Prior Knowledge Anticipatory Set (Create Interest in a Topic) To Illuminate Common
Misconceptions Formative Assessment of Content Knowledge (for purposes of differentiation and
mastery for ALL students) Summative Assessment of Content Knowledge Test Preparation
Survey/Poll Discussion Starter Homework Collection Other
Additional explanation of purpose (optional):

Type of session: Teacher-Paced Student-Paced

Identification of students:
Students will log-in so their individual scores will be available to you after the session
SRS activity will be anonymous
Briefly describe what will happen during the SRS activity (For example: What will students do? What
will the teacher do? What materials are needed? How long will the SRS activity take? The overview should
be a minimum of 2 paragraphs.):

The SRS activity I have created is a pre-test that the students will take to give me information on their
standing in social studies. I will present this on the first day to the students and explain that there will be no
grade for it and that it is just for me to know where they stand. The students will need either a computer or
iPad/tablet to complete the test. No other materials will be required. The teacher will walk around the
classroom monitoring the students and answering questions if needed.

Later in the year, my hopes will be to get the students involved and using Socrative. This site is easy for
teachers and students to use. Im hopeful that later on in the year the students can turn around and make
their own quizzes (approved by the teacher) or use the site to create their own study guides. I can provide

information that will be asked on the quiz and have students create their own questions that consist of a mix
between multiple choice, true/false, and short answer.

Types of questions/prompts (Check all that apply):
Multiple choice Multiple Select True/False Yes/No Short open-ended response
or fill in the blank Longer open-ended response
Provide samples of questions/prompts to be given to students:
1. When was the Declaration of Independence adopted?
2. Who fought in the Civil War?
3. List the Allied and Axis Powers of World War II.

Right/Wrong Answers: Will there be right/wrong answers to these questions? Yes No

Mixed (Some will have correct answers, others will not.)
Immediate corrective feedback:
Will you pre-select correct answers to some or all of the questions and display correct response to the class
after the SRS activity? Yes No
Why or why not?

I am using this assessment tool for the purpose of a pre-test. Due to this, I will not provide the correct
answers since we will be going over the information throughout the year and taking a post-test at the end of
the year.
Use of data: What data will be collected as a result of this activity? How will it be used and by whom?
(For example: Will information collected from this activity be used to award a grade? Will the individual
information collected be shared with students and/or parents to help them monitor individual progress?
Will you discuss the aggregate, anonymous data with the whole class to help them learn? Will you use data
to differentiate instruction for students, if so describe how.)

The data collected from this test will provide an idea of where the students are at in history and see what
they really know and what they need to work on.
Describe what will occur after the SRS activity:

After the SRS activity we will go throughout the year learning about the people/events that were on the test.
Describe your personal learning goal for this activity. (For example: What are you trying that you have
not tried before? What do you hope to learn from this activity? How do you hope it will help students learn?
You must design something that will help you learn something new. Honor system!) :

My activity for the SRS will be a pre-test on history that encompasses questions for grades K-5. However, I
hope to incorporate Socrative later in the year by having students create their own quizzes or use the
website to create their own study guide for future tests/quizzes.
