SOFTWARE DEVELOPMENT AGREEMENT

This Software Development Agreement ("Agreement") is entered


as on -------------, by and between Tech Track ("Software Consultants"),
and Zanana Hospital Bannu ("Customer").

WITNESSETH:

WHEREAS, Customer is desirous of retaining Software Consultants to


perform the services described in this Agreement; and

WHEREAS, Software Consultants desires to perform these services in


accordance with the terms and conditions of this Agreement;

NOW, THEREFORE, Customer and Software Consultants hereby


agree as follows:

HARDWARES + EQUIPMENTS

Hardware and equipments if any required will be provided by


customer for its use upon the recommendation of software consultants
and maintenance will also be on account of customer.

Source code will remain property of Software Consultants and
customer will not further sale the software to any other party i.e.
customer will only use the application for Zanana Hospital Bannu and
not any other subsidiary or merging partners.

The following modules will be part of the software:
1. Medical Every patient who visits the hospital has to get registered prior to getting
Record(OPD) any treatment or investigations done. Registration of patients involves
(120,000) accepting certain general and demographic information about the
patient. The patient is allocated a unique Registration number (MR No)
and a Patient Identification number. The Patient ID will remain same for
his all subsequent visits to the hospital. The OPD charges (if applicable)
can also be collected for the OPD patients during registration and a
receipt will be generated.

The following information is required for the registration of OPD Patient
i. Patient Demographic Details like Name, Age, Sex, Address, Contact
number, Nationality, etc.
ii. Referring Source and Sponsorship / Penal Details
iii. Department or Consultant to be visited.
iv. Thorough MR No patient Dialysis can be track, that how many time
patient came for dialysis and also generate report for patient
Emergency / casualty format will be defined in system
2. Emergency This module consists of Patient Registration form, Patient Visit form,
Management Treatment form, Diagnoses and Prescription forms. All patients at
System Registration Counter OPD are registered using Patient Registration form,
(50,000) while patients are given a unique ID on the basis of which the patient
record can be maintained and updated for admission and different
clinical tests
Revenue report can also be generated using the patient registration
form.
Different kinds of reports i.e. Disease Wise Report, Expired Patient
Report, Age Wise, Gender Wise Report etc can be generated which can
be used for statistical analysis and research work

ACCEPTANCE & DEPLOYMENT

All requirements will be discussed on the customer level; software


Consultants may also provide feedback. But customer will provide a
single person for requirements. And upon deployment all bugs may be
reported likewise.

All deployment will be done on weekly basis via online deployment and
software consultants may also choose to visit physically to deploy the
modules; deployment may be delayed sometimes based on the size of
change-set. Online deployment must be facilitated by the customer to
provide access and credentials; and server must not be used for any
other purpose.

TRAINING

Training will be given to a designated IT member and he will further


provide trainings to end users, however if difficulties are faced, end-
user may communicate to the requirement personnel and then if
required it may be communicated to software Consultants for further
assistance.

MAINTENANCE

Maintenance will include modules mentioned above + minor


developments. (Minor developments include developing new reports,
adding fields and changing formats)

One month free maintenance will be provided to customer from the


day of delivering software.

Afterwards customer will provide the maintenance tasks via email to


software Consultants, software Consultants will provide a time period
for changes and deploy the changes online. Customer can sign
maintenance agreement with consultant.
