PETITION FOR GUARDIANSHIP OF MINOR

COMES NOW, Petitioner unto the Honorable Court, most respectfully avers, as follows:

1. That I, HELEN A. NABAYAN, of legal, Filipino, and a resident of #2L, Nadayao

Road, Brgy. San Pedro, Puerto Princesa City, is the grandmother of two minor
children LOUWIS ALFRED E. NABAYAN 14 years of age, born on August 14,
2002 and LOUWINE DOMINIC E. NABAYAN, 13 years of age, born on August 8,
2003;

2. That the above-named minors are presently residing at ___________;

3. That the above-named minors inherited from their deceased father his undivided
share from a parcel of land covered by Transfer Certificate of Title (TCT) No.
183107, containing a total area of ______________, located at
_________________________;

4. That due to the minority of the said MINOR, it is necessary and convenient that a
guardian over their persons and property be appointed;

5. That, as above stated, Petitioner is the person having the said minor in her care,
and that she possesses all the qualifications of a person;

PRAYER
 WHEREFORE, it is respectfully prayed that, after due notice and hearing,
petitioner be appointed guardian over the estate of the two minor children LOUWIS
ALFRED E. NABAYAN and LOUWINE DOMINIC E. NABAYAN.

Petitioner prays for other relief just and equitable under the premises.
