Social Story SMART Goal for Student K

S In my placement, I was introduced to a 4 year old student who has Autism,


She has an impairment in communication and socialization. She display a
stereotype pattern of behavior by consistently screaming whenever the
teacher is showing video to the class during circle time or doing things out
of turn by walking to the chalk board when she isnt called or making a
humming sound which doesnt produce any words.

My goal is to help her to understand how to self- regulate by feeling safe,


calm and prevent her from running outside when she is scared.

M The measurable outcome for success will be fulfilled once


I can create 4-6 strategies that can help student K to reduce her anxiety.
My evidence will be a Social Story, Happy Box for Sensory needs
As well as a sample student profile, I will create to learn how to observe
behaviors.

A I also spoke to the class teacher and Educational Assistant about student K
and I was given permission. I will do the following to achieve my goals.

1. Research strategies for sensory needs


2. Create a Happy Box to store the sensory tools to keep her safe, calm
and happy.
3. Write a Social Story using Carol Grays Check list, to introduce her Safe
Happy Box with assistance.
4. She will go to the Happy Box willingly without assistance when she is
feeling scared.
5. Possibly tools for the Happy box will be a blanket, headphone weighted
animal to appeal to her sensory needs.

My evidence will be a Social Story, Happy Box and sensory tools

R One barrier that could prevent me from reaching my goal is that I do not
work one to one with student K, Im only shadowing.
However, I will interact with her whenever possible and continue to work on
my goal by talking to the school team.

T My goal started from week 3 and conclude March 24th, 2017
