Adlib compatible FM synth

This is a patch map for older FM synthesizers.
These include the SoundBlaster series (Pro,
SoundBlaster 16). This doesn't include the
SoundBlaster AWE32 and later SoundBlasters.
-The patch map is General MIDI.
-The drum map is General MIDI.
-The Drum Channel is 16 for these FM cards.
This is different than other GM MIDI instruments,
which use channel 10 for drums. So if you don't
hear drums, make sure that you use channel 16
(or vice versa). The channel is set in the Options
| Preferences |Channels dialog.
