JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, NINDA L. GAMPON, of legal age, Filipino citizen, and a

resident of Barangay Bobon 1st, Camiling, Tarlac, Philippines, and
SALLY D. ESPINA, of legal age, Filipino citizen, and a resident of
Barangay Palimbo Proper, Camiling, Tarlac, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and
state THAT:

1. We both personally know ERLINDA DACPANO

ANCHETA (Erlinda, for brevity), born on October 30, 1942 to
Spouses Vicente Ancheta and Susana Dacpano in Agoo, La
Union;

2. Erlinda is also known as MRS. ERLINDA ANCHETA

ESGUERRA after she got married to Teofilo Esguerra;

3. Erlinda had been using NOVEMBER 30, 1942 as her

Date of Birth. As a result, all her transactions and pertinent
documents including her SSS record bear such date of birth;

4. After securing the required documents, it was

discovered that the recorded date of birth of Erlinda
appearing in her Certificate of Birth is OCTOBER 30, 1942;

5. We are not related to Erlinda by affinity or

consanguinity except that of being her neighbor and
friends;

6. We are executing this Affidavit to attest to the fact

that the names ERLINDA ANCHETA, who was born on
October 30, 1942, as appearing in his Certificate of Birth,
ERLINDA ANCHETA ESGUERRA, whose date of birth appears
as November 30, 1942 in the above-said Certificate of
Death including her SSS record, refer to one and the same
person, and that is our neighbor and friend;
7. Finally, we are executing this Affidavit to attest to
the truth of the foregoing statements and for whatever legal
intents and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our

signatures this 27th day of March, 2017 at Camiling, Tarlac,
Philippines.
