PETITION
PETITIONER, thru counsel and to the Honorable Court, alleges:
1. Petitioner, of legal age, and a resident of ________.

2. He executed a last will and testament herein attached as Annex A.

3. The subscribing witnesses to the will of the petitioner are: ________,
_________,_________, all of whom are residents of ____________;

4. The following persons, all of age, have been named in the said will as
heirs, legatees, devisees of the testator:
5. The property owned by the testator that will be distributed upon his death
consist of real and personal property, the gross value of which is________;
and
6. The testator, at the time of the execution of the said will, was ____ years
old, of sound and disposing mind, and not acting under duress, fraud, or
undue influence and was in every respect capacitated to dispose of his
estate by will.

WHEREFORE, it is respectfully prayed that upon proper notice and

hearing, the abovementioned will of the testator be admitted to probate.

Other reliefs just and equitable are likewise prayed for.
