AFFIDAVIT

(Attesting the selling of No. of lots)

Covered OCT. (No.12) P4732
We the heirs of Mudia Clan, Reny S. Quia, Vicky Mudia, Anita Mudia and a
resident of Mudia Village, Labangal General Santos City, Philippines, hereby depose
and say;
We sell a number of lots located at Barangay Labangal, General Santos City a
parcel of land from 973 hectares covered OCT. No. (12) P4732 to DATU AKEEM
GIOVANNI K. PANTAO and his group following applicant names listed attached to this
affidavit and quantity of individual lot resident of Santo Nio Municipality and Cotabato
City.
That the group of Mr. Datu Akeem Giovanni K. Pantao was fully paid the lot rights
to certain BAI HASNA OMAR TIBAN as the coordinator, collector in the payment of lots.
That the amount of per lot is Fifty five thousand pesos only ( 55,000.00)
including the original certificate of titles (OCT). the mode of payment of lot is Five
Hundred Pesos (500.00) per month and upon full payment to the lot by the applicant
the original certificate of title is awarded and given to by the chairman of the Mudia Clan
or by his representative.
That we are executing this affidavit to establish the truth of all the foregoing
statements and for whatever legal intent that this affidavit may serve.

IN WITNESS, WHEREOF, We have hereunto set our hand and affixed our

Labangal, General Santos City.
