Valdosta Middle School STEM Academy

Parent and Student Contract 2017-2018

We understand that:
The student must demonstrate excellent attendance and punctuality throughout the day.
The student must complete rigorous coursework.
The student, in cooperation with parents & counselors, will develop a 4-6 year career and college readiness plan
while enrolled in the VMS STEM Academy.
The student will work toward continuous improvement of their ability to demonstrate the use of 21st Century Skills:
communication, collaboration, critical thinking, and creativity.
The student is responsible for lost, damaged, and destroyed property, electronic devices and textbooks.
The student will adhere to the STEM Academy dress code t-shirt, polo and professional dress day.
We commitment to the following:
The student will behave in a mature, respectful manner, cooperating with teachers, professionals, and students.
The student will obey the rules of the Code of Student Conduct and the School Handbook for Students.
The parent will keep school officials informed when there is an unavoidable conflict.
The parent will communicate any academic needs or abilities to the Principal and/or teachers to ensure the staff is
aware of allowable educational opportunities for the student.
The student will participate in the VMS STEM program for a minimum of 1 full academic year (Aug-May). If a student
chooses to withdraw, he/she will be required to be placed on an appropriate VMS academic team.
The parent agrees to consider volunteer service annually at Valdosta Middle School and will attend events
pertaining to the STEM program.
The student will participate in one or more STEM competitions during the school year such as science fair, math
counts etc.
Academic probationary policies:
The student must maintain a minimum grade of 75 in each core course and demonstrate continuous and consistent
progress.
The student must be eligible for promotion at the beginning of each year in order to remain in the Valdosta Middle
School STEM Academy. There is no probation provided for students who are not eligible for promotion at the end of
a regular school year.
Parents/guardian will be informed of any academic probation and a conference will be held.
After one grading period, the Probation Review Committee will determine if the criteria for continued enrollment
have been met. If such criteria have not been met, the student may be required to withdraw from the STEM
program and return to a grade-level team.

I have read and understand the contract, expectations, policies and commitments. I had an opportunity to ask questions about
the information provided about the Valdosta Middle School STEM Academy. I understand that failure to meet the stated
expectations can result in withdrawal from the Valdosta Middle School STEM Academy. There is an automatic probationary
period for disciplinary violations of the student contract listed above.
