FIELDWORK EXPERIENCES

Level I

January-April 2016 Center for Childhood Development

Under the supervision of Lauren Vetter, OTR/L, I was able to administer and
interpret clinical assessments, plan and administer client-centered
interventions for both individual and group treatment sessions, and utilize
the facilitys charting system to document all sessions. All clients seen in this
setting were pediatric. Primary diagnoses seen included Autism Spectrum
Disorder, Cerebral Palsy, Bipolar Disorder, Dysphagia, Apraxia, and
unspecified regulation disorders.

May-August 2016 Hope Network Behavioral Health Services: Side by Side

Under the supervision of Lorna Dawson, MSW, I was able to provide
occupational therapy services to participants at this day program for older
adults with dementia or Alzheimers. I performed the Allens Cognitive Level
Screen (ACLS) on each participant and designed appropriate activities to suit
each participants cognitive level, thereby enhancing participation. I also
coordinated an in-service for the staff to educate them about the ACLS and
the implications for participants at each level.

September-December 2016 Watervilet Outpatient Rehabilitation Center

Under the supervision of Kristen Hankamp, OTR/L, I was able to administer
and interpret clinical assessments, plan and administer client-centered
interventions, and utilize the facilitys charting system (Epic) to document all
sessions. Primary diagnoses treated included Medial Epicondylitis, Lateral
Epicondylitis, post-CVA, Triceps Tendonitis, post surgical Boxers fracture,
and Elbow Fractures.

Level II

January-March 2017 Spectrum Health United Hospital

Under the supervision of Laura Yonkers, OTR/L, I was able to complete a
placement in subacute rehab and acute care units. I conducted and analyzed
ADL-based evaluations, planned and executed client-centered interventions,
and utilized Cerner (for acute care) and Vision (for subacute rehab) to
document evaluations, daily treatment notes, discharge documentation,
progress notes, and complete billing charges. Other experiences included
conducting co-treatments with physical therapists, providing an in-service to
the rehab staff on the Home Safety Self Assessment Tool, participating in
daily team meetings and patient care conferences, and collaborating with
physical therapists, nurses, and social workers to make discharge
recommendations.

April-June 2017 Mary FreeBed Rehabilitation Hospital

Under the supervision of Jennifer Mazurkiewicz, OTR/L, I completed a
rotation in several units of Mary Freebed. I primarily saw patients in the
pediatric outpatient program, but also had the opportunity to provide
coverage for therapists who work on the inpatient pediatric team, medically-
complex (oncology) team, spinal cord injury team, and brain injury team. I
conducted and analyzed evaluations, planned and executed client-centered
interventions, and utilized Cerner to document evaluations, daily treatments,
discharge documentation, progress notes, and complete billing charges.
Other experiences included compiling a booklet of common visual deficits
seen in patients with brain injuries, their clinical presentations, and
guidelines for treatment. I was able to present this booklet in an in-service to
the inpatient pediatric team.


RESEARCH EXPERIENCE

August 2014-April 2015 Topic: Prosthetic Prosody

Member of Dr. Jennifer Gross research team for 2 semesters. Roles included
compiling background research, developing research hypothesis, creating
research stimuli, writing comprehension questions, counterbalancing
experimental conditions, and developing the experiment using Survey
Monkey.

August-December 2014 Topic: Sensory Integration

Conducted a literature review on the efficacy of various sensory integration
methods used to treat auditory, tactile, and vestibular defensiveness in
children with autism for my honors college senior thesis. This project was
overseen by Dr. Jamie Owen-Deschryver.

September 2015-December 2016 Topic: Multisensory Environments

For my masters research, I was part of a team that conducted survey
research investigating the use of multisensory environments in Michigan
schools. Duties included survey design and implementation, analysis of
qualitative and quantitative data, and compiling article submission for the
Journal of Occupational Therapy, Schools, & Early Intervention.


EMPLOYMENT

2006-present Therapy Shoppe, Inc.

Employed part-time in shipping/receiving, doing website database work,
clerical work, and translating foreign emails.

August 2013-December 2015 Grand Valley State University

Employed at GVSUs Statistics Tutoring Lab. Duties included helping students
with statistics homework and consulting on data analysis.

April-August 2014 Indian Trails Camp

As a skill building mentor and community living support aid, I assisted clients
with improving their independence through community-based activities. I
provided one-on-one support through assisting, reminding, guiding, observing,
and training the clients in a variety of different activities, including
transportation to community events, shopping, physical activity, personal care,
and activities of daily living in the household. I was also responsible for
providing direct care to specific clients. I worked 30-35 hours per week.


COLLEGE ACTIVITIES AND HONORS

Intervarsity Christian Fellowship- Member of this organization for 3 years,
led my own small group for 2 semesters
Frederik Meijer Honors College Ambassador for 2 years. Responsible for
giving honors college tours to prospective students and their parents.
Finalist for the Frederik Meijer Honors College Outstanding Senior Award.

COMMUNITY INVOLVEMENT

Recreation Center Volunteer- Volunteered at GVSUs recreation center for
one semester and worked with high school students with cognitive and
physical impairments to develop person-centered exercise regiments.
Special Olympics Volunteer Coach for 4 years
Lincoln School Volunteered 2-3 days per week in Lincolns Occupational
Therapy Department for 3 summers working with severely, multiply
impaired children. I have volunteered over 250 hours in this setting.
Grand Rapids Domestic Abuse Center Volunteer in their childrens ministry
intermittently since 2009.
Volunteered twice per week for three months at Mary FreeBed in the
outpatient pediatric unit.
