Abstract:
The hospital policy should identify areas in which concentrated electrolytes are clinically
necessary and find how they are clearly labelled and stored in those areas in a manner that
restricts access to prevent inadvertent administration (Joint Commission Resources, 2013). Since
October 2012, our hospital has removed storage of concentrated electrolytes injections outside of
pharmacy with the exception of potassium chloride in the cardiac surgical theatre and magnesium
sulphate in the obstetric ward. Regarding routine potassium replenishment, 74.83% (1392/1860)
of ICU patients received enteral potassium and consumed 88.32% of total potassium chloride
dose for this purpose (25,925g/29,354g), indicating an optimistic adherence rate among
physicians. Since we implemented the stewardship intervention program for safe medication and
potassium chloride injection, adverse events in the patient have not been identified in the
mandatory electronic medication administration error reporting system.
