PETITION

COMES NOW, the undersigned petitioner and to this Honorable Court,

most respectfully alleges that:

1.) He was admitted to the Philippine Bar on April 26, 2007 and was
assigned Attorneys Roll No. 53205;

2.) He is a member of good standing of the Integrated Bar of the

Philippines, Negros Occidental Chapter, and had paid his IBP dues as per
IBP Official Receipt No. 1064564 dated Jan. 18, 2017. He is of good
moral character per Certification issued by IBP Negros Occidental
Chapter enclosed as Annex A;

3.) He has no pending administrative case against him before the Office of
the Bar Confidant as per Certification dated December 28, 2016,
enclosed as Annex B and has no derogatory record as per certification
 from the National Bureau of Investigation dated November 04, 2016,
enclosed as Annex C;

4.) He holds office at the MALUNES MAMPANG MONTORO Law Offices, at

Room 202, 2nd Floor, Parklane Building, Hilado-Tindalo Sts., Capitol
Shopping Center, Bacolod City, with telephone number 709-0080;

5.) He undertakes to immediately inform the Honorable Court of any

change of his office address and/or telephone number and he may
further contacted through telephone number 709-0080;

6.) He now desires to be re-appointed and re commissioned as Notary

Public acting for and in the Cities of Bacolod and Talisay and the
Municipalities of Murcia and Don Salvador Benedicto, Province of
Occidental Negros, for a term effective January 1, 2017 and to end on
December 31, 2018;

7.) He has not been appointed Notary Public by any other Executive Judge
for the term applied for; and

8.) He hereby affixed three (3) facsimile signatures and the true impressions
of his dry seal in Annex D of this petition together with his one (1)
passport size picture at the bottom hereof attached as Annex E.

WHEREFORE, premises considered, it is most respectfully prayed of this

Honorable Court, that the undersigned petitioner be re-appointed and re-
commissioned as Notary Public for and in the Cities of Bacolod and Talisay and
the Municipalities of Murcia and Don Salvador Benedicto, Province of Negros
Occidental, starting January 1, 2017 and ending December 31, 2018.
