SCOPE OF WORKS

OVERVIEW

1.1 The Project

Lend Lease has been commissioned by the Roads and Maritime Services (RMS) to
undertake the Detailed Design and Construction of the Pacific Highway Upgrade
between Oxley Highway and Kundabung, tying in north of Barrys Creek. The Project
involves upgrading the existing two-way Pacific Highway to a dual carriageway
configuration. The Project comprises the construction of approximately 24km of
dual-carriageway highway and associated structures.

2. GENERAL DESCRIPTION OF THE WORKS

2.1 General Description of the Works

This scope of works is comprised of the supply and installation of Concrete Injected
Columns (CICs) to the Project in accordance with this Scope of Works, RMS
Specifications, Australian Standards and drawings listed in this Scope of Works. The
work under the Subcontract includes but is not limited to the ground improvement
works for the following location/s:
Abutment B, northern abutment of the Hastings River Bridge.

3. EXTENT OF THE WORKS

3.1 Inclusions

The works include, but are not limited to, the following:

The Subcontractor must provide sufficient resources to meet the Contractors

obligations, including all equipment, temporary materials, plant, machinery and
personnel.
The Subcontractor is to nominate a Project dedicated Site Supervisor. The Site
Supervisor will be responsible for;
Coordinating the daily and upcoming works with the Contractors
representative;
Dealing with site performance issues;
Attend safety inspections and incident investigation meetings as required;
Liaising with the Contractors representative for all coordination of the
works;
Participate in planning sessions as required; and
The Supervisor will not be permitted to be taken from the Project to work
elsewhere or on another project, unless otherwise agreed in writing by the
Contractor.
The Subcontractor is responsible for the mobilisation, supply, orientation,
maintenance, fuelling and repairs of all plant temporary materials and
equipment associated with the installation of concrete injected columns.
Supply, delivery, unloading and storage of all materials to facilitate the
installation of concrete injected columns to an area designated and allocated by
the Contractor. The Subcontractor will be responsible for the handling and
management of all of the Subcontractors materials from the designated storage
area.
The Subcontractor will conduct all activities to facilitate the safe and efficient
coordination alongside all other works in the area including, but not limited to,
bridge construction, vertical wick drain construction, bulk earthworks and
project haulage operations. The Subcontractor shall provide at least 48 hours
notice of required concrete deliveries where concrete is provided by the
Contractor. Where the Contractor supplies concrete, supply will be via an
agitator truck only.
The Subcontractor must maintain work sites in a clean and tidy manner
throughout the duration of the work; the extended storage of rubbish or loose
items on the construction site or elsewhere is not permitted. Any clean-up work
complete by the Contractor will be charged to the Subcontractor.
Any damage to erosion and sediment controls caused by the Subcontractor must
be rectified immediately with all costs borne by the Subcontractor.
In the event that site issues arise beyond the capabilities of the Subcontractors
Site Supervisor and site crew, the Subcontractor is expected to provide technical
assistance at the request of the Contractor to assist in the resolution of any such
site issue.
Prior to establishing any plant on site, the Subcontractor must provide the
Contractor with its plant loads and bearing pressure requirements for the
working platform and the proposed methodology for those works. The
Contractor will confirm with the Subcontractor the size of the working platform
that will be provided.

 The Subcontractor will submit daily installation records, reports and logs from
the rigs on-board computer to the Contractors representative which include, but
is not limited to, the records required for each CIC as outlined within Section 4.3
of RMS Specification D&C R225. These are to be signed and submitted daily by
the Subcontractors Site Supervisor and Contractors representative. These
signed dockets are required for payment of works completed. Signing of dockets
will not be backdated by the Contractors Representative. It is the
Subcontractors responsibility to ensure these dockets are signed by the
Contractors nominated representative.
It is the Subcontractors responsibility to sequence the works in such a way that
the construction of CICs does not damage or bear any negative impact on
previously installed wick drains or any adjacent CICs. Costs for rectification of
any damaged wick drains or CICs will be borne by the Subcontractor.
The Subcontractor must make all provisions necessary to reduce the height of
spoil concrete on top of the finished CIC while it is still wet and workable.
The Subcontractor is responsible for the protection of the Subcontractors
property, materials and works until the time at which the works are handed
back to the Contractor or are finished in the area, and the Subcontractor has
demobilised from site.
Protection of any other of the Contractors works which may be affected by the
Subcontractor is the responsibility of the Subcontractor. Any damage caused by
the Subcontractor or the Subcontractors work will be rectified by the
Subcontractor with all costs associated with rectification borne by the
Subcontractor.

Installed CICs are to be picked up by the Contractors Surveyors at the end of

each day. It is the responsibility of the Subcontractor to coordinate and manage
this process each day as part of the Subcontractors works.

All works are to be carried out during the normal working hours of Monday to
Friday 7am to 6pm. Saturdays 8am to 1pm are to be worked if requested by the
Contractor.
The Subcontractors rates are deemed to have made allowance for the
Subcontractor to comply with:
The Contractors Safety Management Plan, including but not limited to site
inductions, Safe Work Method Statements, Daily Toolboxes, Job Hazard
Analysis and Permits to Work;
The Contractors requirements for Environment, Health and Safety as
outlined in the document AR503D Lend Lease Engineering
Requirements Environment, Health and Safety;
The Contractors Quality Management Plan; and
The Contractors Construction and Environment Management Plan.
All testing and CIC validation as required within Specification RMS D&C R225,
excluding concrete slump and cylinder strength testing, is to be supplied and
completed by the Subcontractor with all results given to the Contractor in
writing within 24 hours of the test being completed.
The Subcontractor must assess ground conditions onsite and is deemed to have
visited the Site and made necessary allowances to safely and efficiently walk,
track or drive any plant vehicle or equipment on site. The Contractor will not be
responsible in anyway whatsoever should the Subcontractors plant or equipment
become stranded or immobile. All costs associated with salvaging any plant or
equipment shall be borne by the Subcontractor.

3.2 Exclusions

For avoidance of doubt, the following items are specifically excluded from this
Subcontract.

Approval and issuing of all Authority to work, and associated work permits.
Service identification and ground penetration permits. The Subcontractor must
review and ensure they understand where all the services are located. If the
Subcontractor is to strike a service then all repairs, replacement and associated
costs will be borne to the Subcontractor. Note there are underground services in
the surrounding area such as the Sydney to Brisbane optic fibre and local 11kV
underground power supply.
Provision of a surveyor for the purpose of the set out of CIC location control
points and grid patterns. The Subcontractor Supervisor will need to liaise with
the Contractors Representative and provide a minimum 24 hours notice for any
survey requests. Should any damage occur to the control point survey provided,
the costs associated with the re-survey will be borne by the Subcontractor.
Survey asbuilts will be provided by the Contractor and completed daily on works
completed by the Subcontractor. The Subcontractor Supervisor will need to
liaise with the Contractors Representative and ensure survey asbuilts are
complete on a regular basis.
Supply and maintenance of concrete washouts will be carried out by the
Contractor.
The Contractor will manage all spoil associated with the drilling works required
as a part of the construction of CICs. It is the Subcontractors responsibility to
notify the Contractors nominated representative of all unsuitable material
including the presence of any acid sulphate soils found during drilling operations.
The Contractor requires a minimum 24 hours notice to coordinate spoil removal.
Construction of a suitable working platform, providing adequate bearing capacity
as nominated by the Subcontractor for the Subcontractor to complete works
within their scope.
Water cart for wash-down requirements. The Subcontractor shall give the
Contractor a minimum 2 hours notice prior to requiring a water cart.
Concrete Supply for the CIC construction works. The Subcontractor must
provide a minimum 48 hours notice to the Contractor for the planning of
concrete works. The Subcontractors site supervisor must inform the Contractor
of specific details including date, time, load size and load spacing. The
Subcontractors site supervisor must be present onsite to receive all loads. It is
the Subcontractors responsibility to communicate any issues with concrete
supply such that the operation runs efficiently. Concrete orders must be given to
the Contractor a minimum of 24 hours prior to the desired delivery date/time.
Traffic control as required for site deliveries, plant mobilisation and
demobilisation. A minimum of 5 days notice is required to be given by the
Subcontractors representative for the provision of traffic control. Costs
associated with the provision of traffic control should a minimum 5 days notice
not be given will be borne to the Subcontractor.

3.3 Inclement Weather

During inclement weather, the Contractors Superintendent will determine whether

works will continue or be suspended. Such determination will take into account
safety and environmental considerations ahead of any production considerations.
Once the inclement weather has ceased, the Contractors Superintendent shall
determine when the site is safe to recommence works again, such determination will
consider safety and environmental considerations ahead of any production
considerations. During this time, no claims for costs due to delay can be made by
the Subcontractor.

4. SERVICES AND FACILITIES

4.1 Accommodation

(a) The Contractor will provide:

(i) lunch, change and ablution facilities in accordance with the relevant
Code of Practice; and
(ii) at the Contractors Representatives discretion, areas for the erection of
a Subcontractor supplied temporary office and /or Subcontractor
material storage.

(b) The Contractor will not provide telephones, facsimiles or office equipment for
the use of the Subcontractor.

(c) The Subcontractor must:

(i) maintain on a daily basis lunch, change, ablution and any other
facilities (if provided) in a clean and tidy condition;
(ii) maintain storage areas so as not to impede other Head Contract
works; and
(iii) disconnect all electrical appliances outside working hours.

(d) If facilities and storage areas are not maintained to the satisfaction of the
Contractors Representative, he/she may direct others to undertake such
maintenance on behalf of the Subcontractor with all costs so incurred being
to the account of the Subcontractor.

(e) The Subcontractor must not store combustible, hazardous materials or

dangerous goods in any of the facilities or material storage areas. Such
materials must be stored in designated hazmat areas and containers.

4.2 Potable Water

The Contractor will determine locations for and provide a water supply in
accordance with the relevant Code of Practice. Locations may be changed to meet
construction needs at the sole discretion of the Contractors Representatives.

4.3 Lighting and Electricity

(a) The Contractor will provide:

(i) lighting in areas to be used in common with others for access only;
and

 (ii) 240-volt power to the site accommodation facilities provided by the
Contractor with power boards being located as determined by the
Contractor.
(b) The Subcontractor must provide:
(i) all other electrical power to enable the performance of the work under
the Subcontract;
(ii) all task lighting; and
(iii) leads and safety stands.
4.4 Hoisting (Including Cranage)

(a) Where the Contractor has readily available hoisting and lifting equipment on
Site, the Contractor may at its sole discretion assist the Subcontractor with the
unloading of materials delivered to the Site provided reasonable notice is
provided by the Subcontractor to the Contractor of the pending delivery.
(b) In general, the Subcontractor will be responsible for all necessary hoisting and
lifting requirements associated with the work under the Subcontract including
but not limited to, materials delivery, handling and storage.
(c) Where the Subcontractor requires hoisting, including cranage, it is the
Subcontractors responsibility to provide all information regarding the plant
conducting the lift in addition to the items being lifted as per the requirements
of AR618C Lift Permit. It is the Subcontractors responsibility to provide all
necessary information including notice of any lifting a minimum of one (1)
week prior to conducting the lift.
4.5 Signage

No signage advertising the Subcontractor is permitted on Site.

4.6 Scaffolding

The Subcontractor must provide all scaffolds, mobile scaffolds, scissor lifts, etc.
necessary to perform the work and service any plant or machinery under the
Subcontract.

5. SAFETY
(a) Nothing in this clause limits the generality of any other occupational health
and safety provisions of this scope of works document.
(b) The Subcontractor must:
(i) assess risk and develop and implement safe work method statements for
all work activities undertaken by the Subcontractor and provide the safe
work method statement to the Contractor, prior to the commencement of
the work activity;
(ii) conduct daily pre-start meetings with its employees and secondary
subcontractors to identify additional hazards associated with each daily
work activity;
(iii) perform risk assessments on and maintain all construction plant,
including the keeping and provision of records of these activities as
required by the Contractor;
(iv) obtain all certification and registration necessary for the operation of the
construction plant;
(v) provide suitably trained and competent operators for all items of
construction plant;

 (vi) provide all safety clothing and equipment for its employees including but
not limited to safety helmets, boots, glasses, gloves and high visibility
vests;
(vii) ensure that all electrical equipment including leads are tested and tagged
to statutory requirements;
(viii) conduct as a minimum each week one (1) safety inspection, one
(1) activity observation and one (1) safety toolbox involving its
employees;
(ix) participate in debrief meetings in regards to any accident or incident
involving the Subcontractors employees or secondary subcontractors;
(x) ensure that all its employees and secondary subcontractors have
completed both a Site and industry induction course prior to commencing
work on the Site. The Site induction course will be carried out by the
Contractor;
(xi) nominate, where requested by the Contractor, an employee who will
represent the Subcontractor as health and safety representative and
attend Health and Safety Committee meetings; and
(xii)nominate, where requested by the Contractor, a competent
representative of the Subcontractor to undertake health and safety
supervisory training. The training will be carried out by the Contractor
where required.

6. TEMPORARY WORKS
(a) Prior to establishing any plant on site, the Subcontractor must provide the
Contractor with its plant loads and pressure requirements for the working
platform and methodology for those works. The Contractor will confirm with
the Subcontractor the size of the working platform that will be provided.
(b) Once the working platform has been certified, the Subcontractor must not
modify the design, materials or methodology for this or any other Temporary
Works without written instruction from the certifying engineer.
(c) The Subcontractor must ensure that the use of the Temporary Works
complies with and is not used for any other purpose than its intended and
certified use and prior to its application has conducted all necessary
inspection and testing required verifying its compliance.

DOCUMENTS INCLUDED IN THE SUBCONTRACT

Where the documents listed below are not exhibited to this Subcontract, they are
available for inspection from the site office upon request.

In the documents provided that are otherwise attachments to the Head Contract, the
requirements of and references to the Contractor are deemed refer to the Contractor
and its Subcontractors which includes the Subcontractor in reference to all obligations
relevant to the Subcontract Works.

Note (1): All quantities above including spacing, depth and length of treatment are
indicative of the current 85% Geotech Design and will not be locked into the final
Subcontract Agreement. These are subject to change and final Subcontract Agreement
will be finalised on a unit rate of $/m based on depth of CIC.

The Subcontractor confirms that allowance has been made in the rates/price for the
following:

a) Lifting Plant or Excavators are to be fitted with anti-burst valves and all lifting
equipment tagged and certified
b) Prior approval must be obtained from the Contractors representative for any
out of hours servicing. For Safety reasons, no-one is permitted to work alone.
c) Trucks must be fitted with automatic tarps.
d) All plant must be fitted with broadband reversing alarms.
e) Isolating switches are to be fitted to all plant for security reasons.

 f) Costs for the attendance to the site specific induction, toolboxes, pre-starts,
weekly toolboxes and any other Occupational Health and Safety discussion as
directed by the Contractor.
g) All operators must wear seat belts if fitted in the machine.
h) Mobile phones are not to be used whilst operating machinery.
i) Maintenance records will be required at time of plant inspection and prior to
any works being carried out on Site.
