WE, DANNY REQUISO and HELEN DIAMANTE, both Filipinos, of legal ages, married and residents
of Libo, Tuburan, Province of Cebu, after having been duly sworn to in accordance with law do hereby
deposed and state, that

1. We have known Lauriano R. Manlakat and Nancy P. Manlakat as we are neighbors for a long
period of time;

2. Lauriano R. Manlakat and Nancy P. Manlakat have been living together as married couple.
They were married on October 14, 2000 at Tuburan, Cebu;

3. The parents name of LAURAINO MANLAKAT in the marriage certificate recorded on the
Local Civil Registrar and on the National Statistics Office are LOLITA BADIA AND PELAYO
MANLAKAT ;

4. That the correct parents name of LAURAINO MANLAKAT that should appear in the record
Local Civil Registrar and on the National Statistics Office LEONILA ROSAL AND FELIX
MANLAKAT;

5. The parents name of NANCY PESIAO in the marriage certificate recorded on the Local Civil
Registrar and on the National Statistics Office are LEONILA ROSAL AND FELIX PESIAO ;

6. That the correct parents name of LAURAINO MANLAKAT that should appear in the record
Local Civil Registrar and on the National Statistics Office LOLITA GADIA AND PELAYO PESIAO

7. Lauriano R. Manlakat and Nancy P. Manlakat are processing the application for the
correction in the entry of the of Marriage Certificate of Lauriano R. Manlakat and Nancy P.
Manlakat at the Office of Civil Registrar;

8. We attest to the truth and veracity of the foregoing facts and circumstances. That we
execute this affidavit to support the application of Lauriano R. Manlakat and Nancy P.
Manlakat for Correction in their Marriage Certificate at the Office of the Civil Registrar and
for whatever legal purposes this may best serve.
