Step 1
Determine the Sick Meridian(s)
When needling, identify the sick channels the
acupuncture way (not TCM, zang fu diagnosis
way for herbs)
Chinese anatomy identifying the channels.
Yin side and yang side. 3 sections for each side.
Yangming (frontal yang, front of the dark meat),
shaoyang (in between, lateral side), taiyang (major
yang, most yang part, back side of the dark meat).
Taiyin (double yin, frontal white meat), shaoyin (minor
yin, backside of the white meat), jueyin (in between,
medial side)

Case Study: Carpal tunnel


Case: pain shooting through the wrist up to the
middle finger.
Step 1: Determine the sick channel, the
acupuncture way

Step 2: Find a balancing channel
System 1: Foot jueyin (LR) balances hand jueyin

Needle the LR channel where??? Wrist mirrors where
on the LR channel?? Needle the ankle to treat the
wrist. LR4,3,2,1 (cover the whole area because the
problem is a shooting pain to the finger). Needle the

If pain shifts
If the pain also shows up on hand taiyin,
treat the foot taiyin (sp 4,5,2,1).
Step 1: Dx. Hand Taiyin Lu sick
Step 2: Choose balancing ch. Foot Taiyin
Step3: Choose point/location using mirror,
ankle to toe area (SP 4,5,2,1)

Tip
If you dont see immediate effect or
improvement, evaluate where the problem is. It
could be
1. Your diagnosis of the sick channel is wrong
and therefore everything else after that is wrong.
2. You did not pick the correct treatment
channel, or patient does not respond well to that
system. Pick another treatment channel
3. You did not quite pick the right spot according
to imaging and the mirror concept. Palpate again
the ashi point and needle again.

Tip
If the patient says the pain is the same after you
needle, ask Where is the pain now? There
should improvement (if your diagnosis is right
and your selection of point location is right).
Usually, patient will say the pain shifted or
changed place. The pain didnt change place, it
hurted before but is more noticeable now since
the primary pain is gone.
Because if you ask the patient are you better?
they will ususally say the same. 1. they are so
used to saying that or 2. they want more
sympathy or more attention.

When to use mirror vs. image


Use mirror concept when treating limb
problem. Arm treats leg, leg treats arm.
(carpal tunnel syndrome, treat the ankle,
dont needle the neck)
When treating head and trunk problems,
use image concept to locate where to
needle.

Case Study: Knee Pain
Step 1: Knee pain, LR, SP, ST, GB on the right side.
Step 2: (PC, HT) or (PC, TH) can treat all 4 sick channels
LR, SP, ST and GB. No need to use many needles. For
advanced practitioners or those with only a few needles in
your clinic.
System 1: LR(PC), SP(LU), ST(LI), GB(TH)

Step1: Right side shoulder pain, everywhere, all
ROM is about 20-40% only

Tips:
frontal, raising problem, LI is usually sick, sometimes the
lung.
If the problem is lateral, abduction problem, Shaoyang
prolem.
Aduction, taiyang problem.
Reaching to the back can be yangming or taiyang problem.
Under the armpit, rotating cuff problem, its PC or HT
problem

If the chinese names are the same, they
can be used to treat each other.
Example: Hand Taiyang can treat Foot
Taiyang. Foot yangming balance with
hand yangming., etc.
Anatomically, frontal dark meat on the arm
will balance the front dark meat part on the
leg. Posterior balances the posterior part.
Similarity in anatomy.

Nausea, morning sickness, heartburn..we use PC6.
Why? Where does PC6 image? If the neck is the wrist
and elbow is the umbilicus, the epigastric area/chest is
where PC6 is.
Diagnosis for nausea, morning sickness.what is the
sick channel? Ren, KI, ST channels are sick. (ST
because the channels pass by there, not because there
is stomach (organ) problem. Thats zang fu diagnosis for
herbal prescription).
ST, KI need to be balanced.

Neuropathy on the toes
Numbness, tingling, burning pain on all the toes
When treating toes and fingers, forget the channels and
the 3 steps and 5 systems.
Just Mirror the fingers with toes.
Focus on the yin and yang sides of the toes and the in
between. Palm mirrors the bottom of the foot. Dorsal
side of the fingers mirror the dosal side of the toes. The
sides of the fingers mirrors the sides of the toes. Tips
mirros the tips.
How many needles?? As many as it takes, sometimes
10-20.
Map the fingers with the toes. Needle the OPPOSITE
SIDE of the problem.

Diabetes
Lime juice, one each per day for diabetic
patients (not insulin dependent). (dont
take this with the medication at the same
time. 3 hours apart) Cut the lime in half,
squeeze and add water. Do this for at
least 3-6 months. When sugar level
stabilizes, drink a couple of times a week.

After needle insertion, must obtain qi
sensation. Deeper is fine, especially if the
pain is deep.
to 1 hour treatment time.
First time patients, see them at least twice
a week. For acute, excruciating, constant
pain, they need to come in daily until
symptoms alleviate.
