Flexible Spending Accounts - Meritain


Flexible Spending Accounts (FSAs) are available to our Employees to pay for eligible healthcare and dependent care
expenses using pre-tax dollars. By setting aside / using pre-tax dollars to pay for these expenses, you save money by not
having to pay federal, social security, state and local income tax on your contributions to an FSA. Annual maximums are:
Healthcare - $2,550 Annual Maximum
Dependent Care - $5,000 (per household) / $2,500 per spouse (if filing separate tax returns)

2017 Benefits at a Glance
Family Medical Leave Act (FMLA)
FMLA allows eligible employees to take an unpaid leave of absence for generally up to 12 work weeks in any 12 month
period for the birth or adoption of a child, to care for a family member, or if the employee themselves has serious health
condition. An eligible employee is an employee who has been employed by Habasit for at least 12 months and
worked at least 1,250 hours in the year. Certain states may allow for leave in excess of 12 weeks. Habasit will always
adhere to all Federal, State and Local laws.

Accrued but unused PTO must be taken for unpaid leave of absences.
PTO does not accrue while out on leave.

401(k) Plan - Merrill Lynch


Habasit matches dollar for dollar up to the first 6% of contribution. Accounts are vested immediately under the Safe
Harbor program (6 months from benefits eligibility start date). New employees are auto-enrolled at 3% and increased
yearly until reaching 6%.

Group Life Insurance - Sun Life


Basic Life Insurance is provided at no cost to you in an amount equal to two times your base salary. The maximum
coverage volume is $200,000. There are certain age reductions in force at age 65 and up.

Voluntary Life Insurance - Sun Life


You may purchase additional life insurance for yourself (up to 5 times basic annual earnings). If you purchase additional
life insurance on yourself, you may then purchase life insurance on your spouse (up to 50% of your election) and life
insurance on your children. Evidence of Insurability* is required on all amounts elected unless otherwise stated below.
There are certain age reductions in force at age 65 and up.

*Evidence of Insurability is only waived at time of new hire for up to $200,000 on yourself, up to $50,000 on your spouse
at up to the maximum of $10,000 per child.

Short Term Disability - Sun Life


Short Term Disability insurance is provided at not cost to you. If you are unable to work because of a non-work related
injury or illness you may be eligible for the paid Short Term Disability program for up to 26 weeks. This program will run
concurrently with FMLA (if eligible). The wage replacement schedule is as follows:
1 week elimination period
70% of base salary up to 13 weeks of absence (1 year from benefits eligibility start date)
60% base salary up to an additional 13 weeks of absence (1 year from benefits eligibility start date)

Long Term Disability - Sun Life


Long Term Disability insurance is an available option. After 6 months, if you remain unable to work because of a non-
work related injury or illness you may be eligible for the paid Long Term Disability program for up to normal retirement
age. Wages are replaced at a percentage of basic monthly earnings with a salary cap.

Critical Illness/Specified Disease Insurance - Sun Life


If you are diagnosed with a serious medical condition, critical illness and cancer insurance helps reduce daily stress
about money, so you can focus on getting better.
Pays a lump sum benefit (up to a maximum) for a variety of covered conditions such as heart attack, stroke and
cancer
Use the benefit however you see fit to help cover out-of-pocket medical costs or everyday expenses not
covered by your medical insurance
