Health check instructions/pre-requisites

1. APPOINTMENT BOOKING
a. All Health Check Ups are conducted by prior appointments.
b. Our working hours are 08:00 am to 8:00 pm Monday to Saturday and 08:00 am to 2:00 pm on Sunday.

d. An authorization letter from your company & ID Proof is required to avail services paid for by the company.

2. PRIOR PREPARATION INSTRUCTIONS

a. Minimum of 12 hrs fasting (No food, tea, drinks except water) is essential prior to the check up that involves
fasting blood sugar or lipid profile.
b. Refrain from consuming heavy meals, alcohol, smoking, tobacco or other stimulants 24 hrs prior to any health
check-up.
c. Take a good 8-10 hour sleep the previous night.
d. You are requested to bring samples of urine and stool (Urine Approximate 30 ml, Stool 2 spoonfuls) when you
come for check up. You can get sterilized containers from Apollo Clinic, Thane, Front desk between 08.00 am to
08.00 pm or purchase it from any pharmacy shop nearby or you can use home containers after washing with warm
water & soap (properly dried before using).
e. Men are requested to shave their chest to ensure a good ECG and Tread Mill Test.
f. If you are wearing contact lenses, please remove those 2 days prior to Health Check up.
g. Wear loose clothing and easily removable footwear. Avoid wearing jewellery (Necklaces or Chains).
h. If you are diabetic, avoid taking your medicines (Insulin injections or Anti diabetic tablets) on the morning of Health
Check up. Please carry your medicines along which you can have before or after your breakfast in the clinic. Other
regular medication can be taken as instructed by the doctor.
i. Please bring all your medical records (Of past 1 year) and medical accessories like glasses and hearing aid.
j. You may like to come prepared with your list of questions and concerns you wish to ask our medical experts.
k. If you want any additional investigations, procedures or consultations to be done on request or if required as an
outcome of the health check up, please inform us in advance so we schedule the same accordingly; however these
will be billed separately.
l. For females, during menstruation PAP SMEAR test and Urine and Stool analysis cannot be done.
m. Radiations are harmful for your foetus: If you are pregnant or likely to be pregnant, please inform our
Radiographer before the X-Ray.

3. ON DAY OF APPOINTMENT
a. You are requested to reach the Clinic at 08:00 am or at given time to facilitate the smooth process of your check up.
