DECLARATION FOR DETAILS OF OLD ORGANIZATION NPS FUNDS

Case A: In case if employee was earlier working with Punjab National Bank

1. I was working already in Punjab National Bank and my earlier PF no. was
________. I was not registered with NSDL. Please transfer my old PF account
NPS funds to my new PRAN no as generated.
2. I undertake that I will inform the office (PF & Pension department, HO) at
their mail id nps@pnb.co.in after generation of the PRAN for transfer of the
funds from old PF No account to new PF No account.

Case B: In case if employee was earlier working with any other organization having
NPS facility

1. I was working already in __________________________________

and my employee ID there was ________________. I was not registered
with NSDL.
2. I undertake that I will inform the office (PF & Pension department, HO) at
their mail id nps@pnb.co.in in case if my earlier organization transfers the
NPS funds to Punjab National Bank NPS pool account. I also undertake that I
will inform the office (PF & Pension department, HO) at their mail id
nps@pnb.co.in in case if my earlier organization retags the NPS account at
theirs for transfer of the funds.
