RE: Personal Injury Claim

I, Judith Campbell, of the City of Brampton, in the Regional Municipality of Peel,

Province of Ontario, do hereby retain and employ you as my solicitors to represent me in any

matter concerning a personal injury claim, and to take such steps and conduct such proceedings

as you may consider necessary and proper.

I agree to provide along with this signed retainers a financial retainer in the amount of

$1,000.00, which will be held in trust on my behalf.

I understand that I will be billed an hourly basis plus all disbursements made on my

behalf will be charged to me on a count basis.

I understand that during the course of this matter you may render interim accounts for

fees and disbursements and that upon such account being rendered you may deduct the amount

of the account from monies held by you on my behalf and agree that accounts paid more than 30

days after their date will bear interest at 6 percent per annum from their date.

I acknowledge receipt of a copy of this retainer.
