This policy provides guidelines for reimbursement of OPD Expenses (Consultation, prescribed medical test & medicine bills) due to illness/Diseases/Injury sustained to an employee and/or their family within India. SCOPE & APPLICABILITY 4.1, These rules shall applicable to following category of employees of Sistema Shyam Teleservices Limited (The Company) 4.1.1. CEO & President, Dy. CEO’s 4.1.2. CXO's 4.1.3. Advisors to the president 5. DETAILS OF BENE T 5.1. Coverage 5.11. ‘The maximum coverage limit of an employee & his / her family shall be lacs for a calendar year. 5.1.2, The above sum is on family floater basis. 5.1.3. Family is defined as self, spouse & children. 5.2. Benefit 5.2.1, These benefits are applicable only if the expenses are incurred in prescribed hospital/nursing home as outdoor patient. 5.3. Coverage of Illness 5.3.1. All type illnesses which require outdoor treatment. 5.3.2. Expenses relating to consultation, medicines, and medical tests. 5.3.3. Dental treatment up to Rs.25, 000/- per family per annum (including routine dental treatment), 5.4. Following shall not be covered, 5.4.1. Any surgery Any treatment other than allopath such as Ayurvedic, Homeopathy, Naturopathy, Physiotherapy ete. 5.4.4. Cosmetic or aesthetic procedure. 5.4.5. Critical illness includes — Cancer, Coronary artery, Kidney failure, Major organ transplant, Multiple sclerosis, Stroke. Paralysis and Aids. . HOSPITAL/ NURSING HOMES 6.1, Only Hospital situated in Delhi & NCR shall be eligible for providing OPD Services, 6.2. Maximum three Hospitals / Nursing Homes ean be taken into companies Panel. 6.3. CHRO shall approve the Hospital / Nursing Home on the basis of their reputation, Infrastructure and convenience of the employees 7. PROCEDURE FOR AVAILING BENEFIT: 7.1. Alleligible employees & their Fa prescribed Hospital/ Nursing Homes: Members shall be issued Medical Identity Cards by the 7.2. All medical cards holders may avail cashless access services through the prescribed Hospital /nursing homes. 7.3. In following circumstances the employee may claim reimbursement of medical expenses. 7.4, Medical Expanses occurred while on traveling 7.4.1. In case an employee or his/ her dependent do not carry the card to the prescribed hospital, he/she will have to pay the medical amount to the Hospital, / Nursing Home, which can be reimbursed later on by the company ( within one month of the treatment). 7.5. Employee has to obtain service of the prescribed Hospitals/ Nursing Homes to get reimbursement under this policy. 8. PROCESS OF SUBMISSION OF CLAIM DOCUMENT: 8.1. Employees should submit the claim within 7 days from the treatment completion date. 8.2. All re-imbursement claims required approval of his / her Reporting Manager. All claims will be submitted to the accounts department, dully approved & with all proofs of expenses. 8.4, Following document has to be submitted for elaim: 8.4.1. OPD / Consultation fees receipted (original Bills/Cash memos/reports). 8.4.2. Photo copy of Medical Prescription 8.4.3, Any other information required by Company. 8.5. Conditions: 8.5.1. No payment, exceeding the capital sum, shall be payable 8.5.2. The company has the right to withdraw the benefit to the any eligible employee. 8.6. Refund of the claims: 8.6.1. The claim Coordinator will coordinate claims with Accounts and to get the claim processed. 8.6.2. Claim reimbursement will be in the name of Employee only. 8.6.3. The payment will be credited to the employees reimbursement account (salary account in case of no re-imbursement account provided by the employee). oes fanuary 8.7.
