1. What are the common behavior issues in the school?

- The common behavior issues in the school are punching and bullying. The students
tend to fight all the time on the simple things.

2. What are the consequences of each behavior issue? Describe the policies and
procedures.

- We usually call the attention of student/s involved, and in worse cases, we call the
attention of the parents/guardians.

3. How does the school provide Positive Behavior Intervention Support (PBIS) to
students behavior?

- In bullying, upon the filling of a complaint upon notice by a school personnel or official
of any bullying or peer abuse incident, the same shall be immediately reported to the
school Head, who shall inform the parents or guardian of the victim and the offending
child, in a meeting called for the purpose.

4. How does the school implement programs/activities promoting the Child Protection
Policy (DepEd Order No. 40, s. 2012)? Identify who are involved, frequency, etc.

- The school conducts meetings and seminars for parents on how to discipline their
children. The persons involved are the school administrators, parents, teachers and
students.

5. What are the roles of parents and other stakeholders in promoting positive
behavior?

- Some parents do not attend meetings when attention is being called.


6. Are there any provisions for students with special needs? If yes, state the relevant
policies and procedures.
- N/A

1. What is the common inappropriate behavior of students in the classroom?

- Punching
- Pulling of hair for girls
- Pointing sharp objects like pencil and scissors

2. How does the teacher handle the behavior of the students? (Describe the strategies, PBIS if
any)

A. Appropriate behavior

- Giving rewards like crackers, stamps or stickers

B. Inappropriate behavior

- Say sorry to his/her classmates and shake their hands


- If cases are worse, call the attention of the parents
