Family planning allows women to make informed choices
regarding when and if they decide to have children. It is achieved
through use of contraceptive methods and the treatment of infertility.
Family planning/contraception reduces the need for abortion,
especially unsafe abortion. Family planning reinforces peoples rights
to determine the number and spacing of their children. By preventing
unintended pregnancy, family planning /contraception prevents deaths
of mothers and children. It also benefits to prevent pregnancy-related
health risks in women, reducing infant mortality, helping to prevent
HIV/AIDS, empowering people and enhancing education, reducing
adolescent pregnancies, and slowing growth population.
As if I am a midwife or a family planning counselor, I have done
counseling my parents about the family planning method that is suitable
and accurate to their health status. The first thing that Ive done is to
greet them and introduce myself to establish rapport and making sure
that client is comfortable and assuring them that what we had talked
about will be kept confidentiality. Ive also encourage them to ask
questions if there are some problems that they dont know so that I can
explain it well. Next is to assess what client knows about family
planning and what are the needs and wants to learn about the method.
Based on the data Ive gathered from the assessment, the findings were
that their health status and reproductive health with the help of Family
Planning Service Record, from the World Health Organization Medical
Eligibility Criteria (WHO MEC Wheel) was also helpful to determine
the appropriateness of the client for using the methods in family
planning. Their reproductive health status was in good condition and
they are applicable for all the family planning. I asked them if they
would like to have another baby, and they answered that having another
baby is rejected because they already have 3 children and she have
undergone 3 caesarian sections (CS). The Obstetrician advised my
mother after when she delivered their first baby was to take pills because
she undergone CS and it is to prevent getting pregnant again. Next is to
assess that they are at risk of having STI, and based on the result they
are not at risk of having STI. But there are some methods which can
affect the health of an individual and disadvantages to them. Ive also
asked the relationship with his husband, she answered it that they are
doing fine and there is no problem with that. Next is to inform them
what methods are applicable for them. For me, Ive introduce first the
definition of the permanent method which is tubal ligation. Ive explained
that it is often the best contraceptive choice when desired family size has
been achieved. Ive explained its effectiveness that it is not 100% effective at
preventing pregnancy. Sterilisation is a surgical intervention that provides
permanent contraception for women who do not want any more children. It is
a safe and simple surgical procedure. It is also known as Tubal Ligation (TL),
or tying the tubes. And also I discussed the advantages about the tubal
ligation. It allows you to be sexually active without worrying about becoming
pregnant. Although this is expensive it is a one-time procedure that is safe,
and relatively straightforward to do for a trained person. And sterilisation has
worked you dont have a constant use of a contraceptive method, regular
visits to health facilities or repeated expenditure on contraceptive supplies.
Ive also discussed the disadvantages like the tubes may rejoin and the
women will be fertile again, it may not be reversible, resulting in possible
regret, and it does not protect against sexually-transmitted infections (STIs),
including HIV/AIDS and it takes between four weeks and at least three
months for sterilization to be effective. Ive also introduced the use of male
condoms, effectiveness, advantages & disadvantages. First is I defined what
condom is and how it is used. Condoms are small, thin pouches made of latex
(rubber), plastic (polyurethane, nitrile, or polyisoprene) or lambskin that
cover your penis during sex and collect semen (cum). Condoms stop sperm
from getting into the vagina, so sperm can't meet up with an egg and cause
pregnancy. If it is used correctly, 99% effectiveness is guaranteed. The better
you are about using condoms correctly every time you have sex, the better
they'll work. But there's a small chance that you will get pregnant even if you
always use them the right way. Weve also discussed the advantages of using
condoms, for male condoms these were that when using condoms it doesnt
cost much and are convenient, you don't need a prescription or ID to buy
them, and they're inexpensive and sometimes free, it can be carried easily and
discreetly by men and women, best means currently available of protection
against STIs, including HIV, by preventing STIs, condoms can protect
fertility, it can also prevent cervical cancer, it catches semen so nothing drips
from the vagina after intercourse, it has minimal side effects and it does not
affect menstrual cycle. The disadvantages of using male condoms are that it
requires interruption of sexual activity, it can be torn incidentally by
fingernails and can burst during coitus, and if the wrong size is used, it can
slip off and cannot offer any protection. That was the advantages and
disadvantages that Ive told them for them to be informed and help them to
decide whether to use it or not. During our discussion, I asked them if they
understand it well and they replied yes and I also told them that family
planning provides many benefits to mother, father, children and family. I also
asked them if there is any information that they want to know and that are not
yet clear to them, and they answered that they dont have any more questions
and everything is clear to them. And the next thing to do is to help them make
their decision whether they would use the family planning method Ive
discussed to them. Then if they want contraceptives that are available and
very affordable, I refer them to Rural Health Unit (RHU) and sometimes
there are free contraceptives and you dont need to buy it in an expensive
price. And I also explained that if they have a sexual intercourse without
using the method which is the male condom, the spouse might be a
possibility to become pregnant. Lastly after they choose the family planning
method Ive introduced to them I will let them return for revisits so that I
would know if they used the method or having any difficulties or any
discomforts and other questions with the method. During visits, I would
know the positive behaviors and the success or if they are satisfied with
the method of using male condoms or tubal ligation for female. Then if
they are not satisfied or there are some difficulties to them, I would
introduce other family planning methods which may help them.
