DRIVER'S LICENSE SUPPORTING DOCUMENTS PER TRANSACTIONS AS PER

A. STUDENT PERMIT TRANSACTIONS

1. New Student Permit

Properly Accomplished Application for Drivers License


Original Birth Certificate with Photocopy
- NSO Authenticated Birth Certificate with Official Receipt
Medical Certificate issued by any licensed physician
practicing in the Philippines

Qualifications:

1. Must be at least 17 years old.


2. Must be physically and mentally fit to operate motor
vehicle.
3. Must be able to read and write Filipino, English or the
applicable local dialect.

Procedure:

1. Proceed to the Customer Service Counter to get your


checklist of requirements and secure a Driver's License
Application Form. (This form is available for download).
You can accomplish this form prior to transacting your
business at the LTO. Get a queue number and wait for your
number to be called.
2. When your number is called, proceed to evaluator counter
and submit all the required documents and have it checked for
completeness and authenticity.
3. Proceed to the Photo taking / Signature Area to have your
picture and signature taken when your name is called.
4. Proceed to the Cashier when your name is called to pay the
necessary fees and obtain an Official Receipt.
5. Proceed to the Realeasing Counter when your name is called
and present the Official Receipt and claim the Student Driver's
Permit. You are required to write your name
and affix your signature in the appropriate Release Form.

Venue: Any Licensing Center / Extension Office / District

2. Student Permit - Foreigners

Properly Accomplished Application for Drivers License


Original Birth Certificate with Photocopy
- NSO Authenticated Birth Certificate
Medical Certificate issued by any licensed physician
practicing in the Philippines

Alien Certificate of Registration I-card

Qualifications:

1. Must be at least 18 years old.


2. Must know how to read ans write in English or Filipino.
3. Must be physically and mentally fit to operate a motor
vehicle.
4. Must have arrived in the Philippines for at least one (1)
month prior to the date of application with LTO and can show
evidence from BID/DFA that he will stay
in the country for at least one (1) year valid visa from the
date of application.

Procedure:

1. Proceed to the Customer Service Counter to get your


checklist of requirements and secure a Driver's License
Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business
at the LTO). Get a queue number and wait for your number to
be called.
2. When your number is called, proceed to evaluator counter
and submit all the required documents and have it checked for
completeness and authenticity.
3. Proceed to the Photo taking / Signature Area to have your
picture and signature taken when your name is called.
4. Proceed to the Cashier when your name is called to pay the
necessary fees and obtain an Official Receipt.
5. Proceed to the Releasing Counter when your name is called
and present the Official Receipt and claim the Student Driver's
Permit. You are required to
write your nameand affix your signature in the appropriate
Release form.

B. NEW LICENSE TRANSACTIONS

1. New Drivers License Non Professional

Properly Accomplished Application for Drivers License


Medical Certificate issued by any licensed physician
practicing in the Philippines
Valid Student Permit

Qualifications:

1. Must be at least eighteen (18) years old.


2. Must be physically and mentally fit to operate a motor
vehicle.
3. Must be a holder of a valid Student Permit (SP) issued at
least thirty (30) days prior to the application.
4. Must be able to read and write in Filipino or English.
5. Must be clean, neat and presentable (wearing of sando,
playing shorts and slippers during photo-taking and
examinations will not be allowed).
6. For Foreigners, the applicant must have been in the
Philippines for at least one (1) month with the proof that
he/she will stay in the country for at least one (1) year from
date of application.

Procedures:

1. Proceed to the Customer Service Counter to get your


checklist of requirements and secure a Driver's License
Application Form. (This form is available for download. You
can accomplish this from prior to transacting your business
at the LTO). Get a queue number an wait for your number to
be called.
2. When your number is called, proceed to evaluator counter
and submit all the required documents and have it checked for
completeness and authenticity.
3. Proceed to the Photo taking / Signature Area to have your
picture and signature taken when your name is called.
4. Proceed to the Cashier when your name is called to pay the
application fee.
5. Proceed to the Examination Room for the lecture and
written exam.
6. After passing the written examination, wait for your name
to be called for the practical exam.
7. After passing the practical examination, proceed to the
Cashier when your name is called, present the Official Receipt
and claim the Card type license. You are required
to write your name and affix your signature in the
appropriate Released Form.

2. New Drivers License Non Professional, Filipino w/ valid

6. New Drivers License Professional

Qualifications:

1. Must be at least eighteen (18) years old.


2. Must be physically and mentally fit to operate a motor
vehicle.
3. For RC 1, 2, 4 and 6 - must be a holder of valid Student
Permit (SP) issued at least six (6) months prior to the
application.
For RC 3, 5, 7 and 8 - must be a holder of valid Non-
Professional Driver's License.
4. Must not be afflicted with any contagious disease or
epilepsy.
5. Must be able to read and write in Filipino or English.
6. Must be clean, neat and presentable.
7. Must not have been cited for two (2) or more counts of
reckless driving during the period of validity of his/her existing
license.
8. For foreigners, the applicant must have been in the
Philippines for at least one (1) month with proof that he/she
will stay in the country for at least one (1) year from date
of applciation and is allowed to work herein.

Procedures:

1. Proceed to the Customer Service Counter to get your


checklist of requirements and secure a Driver's License
Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business
at the LTO). Get a queue number and wait for your number to
be called.
2. When your number is called, proceed to evaluator counter
and submit all the required documents and have it checked for
completeness and authenticity.
3. Porceed to the Photo taking / Signature Area to have your
picture and sugnature taken when your name is called.
4. Proceed to the Cashier when your name is called to pay the
application fee.
5. Proceed to undergo written examination in the form of the
PDL Basic Driving Theory Test for the RC/s applied for or
Conductor's License Examinations and will be
immediately informed of the results of the same.
6. Execpt for the application for Conductor's License, an
applicant who passes the written examinstaion shall be
required to immediately take the practical driving test for
theRC's applied for.
After passing both examinations, the application shall be
approved by the Approving Officer.

Applicants who fail to pass the Conductor's License


Examinations, Basic Theory Test and/or Practical Driving
Test shall not be allowed to take the same test within a
period of one (1) month from the date of the last
examination.
If the applicant fails to pass any of the written or practical
examinations twice within a period of one (1) year, he/she
shall not be allowed to apply for the same within a
period of one (1) year from the date of last examination.
If the applicant fails to pass any of the written or practical
examinations thrice, he/she shall no longer be allowed to
apply fo the same for a period of two (2) years
from the time of the lates failure.
In case the application is only for addtional RC/s, she/she
shall be allowed to renew his/her PDL or Conductor's License
and, for this purpose, such shall be annotated
on the record of the applicant.

7. After passing the practical examination, proceed to the


Cashier when your name is called for payment of necessary
fees, and obtain an Official Receipt.
8. Proceed to the Releasing Counter, present the Official
Receipt and claim the Card type License. Yor are required to
write your name and affix your signature in the
appropriate Released Form.

Driver's License Transactions.

7. New Drivers License Professional, Filipino w/ valid
