I, PSSUPT ERNESTO TUBALE BARLAM, of legal age, married, Filipino, presently assigned
with the Regional Investigation and Detective Management Division, NCRPO and with
residence at #245 Moriones, Tondo, Manila, do hereby name, appoint and constitute, NUP
Ricardo Consigna Gonzalez, of legal age, and with residence at Blk 3, Lot 2, Phase 2, EP
Village, Brgy. Pinagsama, Taguig City, to be my true and lawful attorney-in-fact for me and in
my name, place and stead, to do and perform the following acts and deeds;

"To make, sign, execute, and deliver documents and other writings in my name relative to my
withdrawal of Fifty five thousand Pesos (Php 55,000.00) from my Capital Contribution Account
No. 62000107326153 at Armed Forces and Police Savings & Loan Association, Inc. (AFPSLAI).

HEREBY GIVING AND GRANTING unto my said attorney-in-fact, full power and authority to do
and perform every lawful act and thing whatsoever requisite or necessary or proper to be done
in the premises, as fully to all intents and purposes as we ought or could lawfully do if personally
present, and hereby ratifying and conforming all our said Attorney-in-fact shall do or cause to be
done under and by virtue of these presents.
