Discharge Plan

Patient's information:

o Gender : Male
o Admission date : October 15, 2015
o Discharge date : October 18, 2015
o Chief complain/s : dyspnea, puffy eyelids, flank pain, tea-colored urine,
scabies
o Diagnosis : Acute Glomerulonephritis
o Attending physician : Dr. Q

Diet
Instruct significant others to the following:

Low Sodium fruits and vegetables


(Ex:Sweet potatoes, bananas, apples, watermelons,beans,sweet potatoes,
cabbage)
Dairy products
(Ex:,fat free or low fat yougart,cheese, pasteurized milk.
Low Protein Diet
(6 ounces daily of any fresh meats or frozen pork, beef and fish without salt)
Low Potassium Diet
(Ex: unsalted corn, carrots, eggplant, pasta)
Easily digested foods such as cereals, boiled vegetables
(mashed potato, carrots, squash, etc.)
Fruits
(fruit juice, fresh fruits such as orange, apple, ripe banana, grapes and etc.)
Condiments and extras
(Ex:lemon,jams and jellies)
Instruct significant others to AVOID the following:

dairy products that have not been pasteurized


soda, chocolate milk, buttermilk
bacon (all types), sausages or hotdogs
smoked or pickled fish
uncooked grain products
herbal supplements
raw honey

Hygiene
Instruct the significant others to do the following:

a. Observe proper hygiene like taking a bath every day.


b. Brush the clients teeth 3x a day.
c. Hand washing before and after meal.
d. Maintain safe, clean, comfortable and calm environment.
e. Educate the client and family regarding the infection control precaution.

Activity
a) Encourage client/significant others to continue activity in daily living as
tolerated prior to illness, but is necessary once the patient feels well.

b) Advise the significant others to encourage the client to rest and sleep (6-8
hours).

Medications
Instruct significant others to have compliance in taking:

o Multivitamins(Centrum Kids Chewable,flinstones)once a day;daily (take


after consuming a healthy breakfast)

o Benzyl Benzoate emulsion(a cleaner to use and has more rapid effect)

o Calamine lotion if there is itchiness.


Community Support
Instruct significant others to the following:

a. Have accessibility of health center.


b. Avail the health services offered by the nearest health center such as
immunization and check-ups.

Spiritual
a. Counseling: Tell the significant others to pray, it will help clients recovery.
b. Advise the client/significant others to have a strong faith to God.
c. Encourage relatives or family members to provide moral support and widen
their understanding.
d. Advise family to continue normal way of living, not to be discouraged.

Financial resources
a) Instruct the significant others to avail the programs of the government such
as, PhilHealth and SWA.

b) Seek financial assistance to relatives to give for medications and hospital


bills.

Preventive to divert the attention of
the client. (Ex. singing
nursery rhymes, toys,
storytelling)

other to help the client
relax by taking them in a
calm voice, allaying their
fears, and perhaps
covering a back rub or a
massage or other
soothing activity.

Scabies Promotive/Preventive Instruct the significant


others to maintain clean
environment.
Instruct the significant
others to promote the
clients good personal
hygiene.(Ex:Daily bath)
Instruct the significant
others to encourage the
client eat foods rich in
Vitamin A and Vitamin
C such as green leafy
vegetables and fruits.
