CONTRACT OF PROFESSIONAL LEGAL SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Legal Services is made and executed into by and between:

_____________________________, a duly organized and existing Public Corporation

under the laws of the Republic of the Philippines, with principal office address at
________________________________________________, represented herein by its
_____________________________________, of legal age, Filipino, married/single and a
resident of ___________________________________________________, Philippines,
hereinafter referred to as the FIRST PARTY;

-AND-

__________________________________, of legal age, Filipino, married and a resident

of ______________________________________ hereinafter referred to as the SECOND
PARTY;
- WITNESSETH

That COA Circular No. 2012-005 dated December 7, 2012 was issued by the Commission on
Audit requiring the Barangay Captains or Purong Barangay to issue Punong Barangay
Certification (PBC) on all checks issued by the Barangay Government in payment of all their
disbursements;

That the above COA circular requires all the Punong Barangay Certification or PBC to be
notarized by a Notary Public;

That in compliance of the said COA circular, the First Party through the LIGA ng mga
Barangay of the Municipality of __________________________, on its Liga Meeting/Congress
held last ________________ at _____________________, jointly and unanimously hired the
professional legal services of the Second Party to notarize all their PBC issued in payment of
their barangay obligation;

That the Second Party accepted the engagement offered by the First Party and further
offered his legal services to all the barangays of the Municipality of
____________________________

That in consideration thereof, the parties agreed for a monthly retainers fee of
____________________________________, subject to the COA rules and regulations;

That this contract takes effect effective _____________________ and will continue to be in
full force and effect until terminated or revoked.
