w/SherwinWilliams E67BF1704 & Axalta Imron Elite EY BC/CC. Condition samples 72 hrs prior to
testing.

A completed Paccar Paint Test Submission Form is required to begin testing. Digital photos of any tests
that do not meet requirements will be included in the test report.

Note 1: Price includes 1 photo of representative sample prior to testing. Additional charges will be
incurred if re-test for dry film thickness is required.

This quote is valid for 30 days. Photos of tested samples provided upon special request only, extra charges
will apply. Extra charges will also apply if cutting of samples is required to make appropriate number of
test pieces. Please note that ACT does not put a PASS/FAIL rating on reports UNLESS the rating scale is
pass/fail. We will only report our observations.

Test completion is expected to be within 6 weeks from receipt of samples and purchase order. This
completion date is dependent on laboratory capacity at the time of sample delivery and credit
approval. Progress billing will apply for any test with duration longer than 30 days and invoices will be
issued every month. ACT has a $200.00 minimum lab charge. This lab charge will be applied if testing is
stopped early per customer request and total charges of testing actually performed are less than $200.00.
Quote does not include freight charges for return of tested samples to customer. Samples will be returned
to customer PREPAID AND CHARGE unless customer provides a freight account number or specifies to
dispose of tested samples after testing is complete.
Your samples and purchase order should be sent to my attention at the address on this letterhead. Please
reference this quote number on the package.
