Honesty and Responsibility Statement for the Performing Arts

I, ______________________, adhere to the guidelines officiated by the Non-music Major

Orchestra in regards to my participation within a performing ensemble at a venue. By doing so,
I am invoking a claim of personal honesty and responsibility.

Contract guidelines include, but are not limited to, the following:

1. The participant understands that his or her attendance and participation pivotally
contributes to the reputation of the Non-music Major Orchestra as an organization.
2. The participant must have sight-read and played through the ensemble music within the
set list at least once before the performance, both alone and with the ensemble.
3. The participant must provide his or her own instrument, stand, and accessories such as
pencils and rock stops to the venue(s).
4. The participant must arrive at the venue in a timely and punctual manner.
5. Unless transportation is negotiated and provided by the Non-music Major Orchestra,
the participant must provide his or her own transportation. Lack of transportation is by
no means an acceptable excuse for absence.
6. The participant must abide by his or her commitment in regards to attending the
performance event(s). Exceptions include the following: medical emergency, sudden
family death or illness, auto accident, religious holiday or observance, court order,
military deployment.
7. The participant is aware that desertion or disbandment affects every player within his or
her assigned ensemble as an incomplete ensemble cannot perform. Every player is
dependent on each other, so the participant is expected to be committed.
8. In addition, the participant understands that desertion or disbandment brings a
reputation of dishonor and unreliability to the Non-music Major Orchestra.

By affixing my signature below, I agree to uphold and abide by all contract guidelines detailed in
this contract as officiated by the Non-music Major Orchestra. Furthermore, I am aware that the
violation of a guideline will result in the nullification of my honesty and responsibility claim,
consequentially invalidating my own reputation with the organization.
