LETTER OF REQUEST

Dear Ms. Blge,

Good day!

I, EMILIO REYES, of legal age, Filipino, married, and a resident of Veraville Alegrea
Paraiso Drive, Talon 2, Las Pinas City, Metro Manila, am the holder of NIS Yellow Book issued
by the NFD Manning Agency and LMS Manning Agency.

Sometime in November 2013, I lost the above NIS Yellow Book when the Typhoon
Yolanda devastated the Philippines.

Despite diligent efforts exerted, I could no longer locate, find or recover the said NIS
Yellow Book , and to the best of my knowledge, the same is truly lost. Further, a copy of the
aforementioned document can no longer be obtained from NFD Manning Agency and LMS
Manning Agency by reason of the agencys closure.

As such, I am writing this personal letter of request to support my application for the
issuance of a new NIS Yellow Book, in lieu of the one that was lost.
