1.0 INTRODUCTION

1.1 Project Description


This HASAP has been developed for engineering support at the Lima Husky
Refinery in Lima, Ohio. Key contributors include Jacobs Engineers, Lima Husky
Refinery Project Management, Jacobs HSE and Lima Husky Refinery HSE. This
HASAP will be reviewed and revised as conditions or circumstances change
throughout the course of our work at the refinery.
The project scope involves engineering support which includes inspections and
assessments in operating process areas.

1.2 HSE Policy


The Jacobs project team is committed to safety excellence for our employees, its
client, subcontractors and others who may be affected by our work activities.
Commitment and adherence to all applicable health, safety and environment policies
and procedures is a condition of participation in this project and is a cornerstone of
our business relationship with the employees at the Lima Husky Refinery.
Practicing safe work habits, as prescribed in Jacobs HSE procedures, our clients
safe work procedures and all other applicable regulatory and ancillary standards is a
condition of employment. Project personnel are expected to follow these
requirements at all times. Each employee is responsible for his/her safety, as well
as the safety of their co-workers.
The most stringent policy shall apply in cases where policies conflict. Exceptions for
any reason, including previous practice, economic considerations, etc. will not be
considered.
The hazard analysis section of this HASAP establishes the minimum acceptable
safety standards and is intended to address known or potential hazards within the
scope of work. It does not relieve Jacobs from its responsibility to address hazards
that have not been identified in the hazard analysis.

1.3 Objectives
The primary HSE objectives are as follows:
Demonstrate superior HSE performance through our HSE processes and
their successful implementation.
Gain recognition from the Lima Husky Refinery Project Team for
outstanding HSE performance.
Demonstrate exceptional safety leadership traits in all actions taken each
day.
Positively influence the client and their contractor community through our
commitment and passion for safety, demonstrated safety leadership and our
HSE processes and procedures.
2.0 PROJECT SCOPE AND SCHEDULE

2.1 Description of Work


The general work plan description is as follows:
Conduct site surveys and assessments.
Obtain measurements as required.

Access areas to evaluate existing conditions and equipment in order to


develop engineering deliverables

2.2 Schedule
Project start:
Work Hours: Monday through Friday 7:30 a.m. 5:00 p.m.

2.3 Project Manpower


The project resource level for personnel fluctuates as defined by Client
requirements.

3.0 CONTRACT AND REGULATORY HSE REQUIREMENTS AND RESPONSIBILIES

3.1 Lime Husky Refinery and Jacobs Requirements


Specific requirements and expectations are identified in the following documents
and/or through the following regulatory agencies.
Lima Husky Refinery Contractor Policies and Procedures Manual
Jacobs Engineering Health, Safety and Environment Procedures (HSEPs)
OSHA 1910 and 1926 standards
Environmental Protection Agency

3.2 HSE Roles and Responsibilities


Personnel have the responsibility to actively participate in the HSE process and
stop, or bring to the attention of management any at risk behavior, unsafe condition
and/or potentially harmful environmental practice. Imminent danger situations must
be corrected immediately.
Additionally, specific HSE roles and responsibilities are provided below.

Jacobs Project Personnel Responsibilities


Take personal responsibility for their health and safety and the health and
safety of others.
Comply with health, safety and environment policies and procedures.
Suggest improvements in methods or procedures that will prevent
incidents and protect the environment.
Stop, or bring to the attention of management, any unsafe act or condition
and/or any potentially harmful environmental practice.
Actively participate in the development of SPAs.
Participate in the Safety Observation report process.
Immediately correct any imminent danger situations.
Notify supervision if they have an insufficient understanding of the task to
be performed, of HSE procedures and of assigned safety equipment.
Promptly report incidents involving personnel, equipment, property or the
environment to their supervisor, no matter how minor.
Learn the approved HSE safe practices that apply to their work and
practice them at all times.

Ask for assistance from a supervisor when unsure of a work or safety


practice.
Never bypass the function of a safety device.
Promptly report safety device malfunctions to supervision and tag the
device to warn others of the hazard.
Never attempt to do a job alone when good judgment indicates assistance is
needed.
Inspect, use and maintain personal protective equipment that has been provided.
Do not participate in clowning, practical joking or horseplay of any kind.

Jacobs Corporate HSE Manager Responsibilities


Advise management and supervisory personnel in safe work practices and
provide information required to comply with the applicable HSE policies and
procedures.
Assist project management in the development of the Hazard Assessment Safety
Action Plan (HASAP) and ensure it is updated as required.
Ensure HSE records are maintained and submitted when required.
Periodically perform audits and report findings/recommendations to
management.
Deliver HSE training as needed.
Review incident reports and recommend specific corrective measures.

5.0 PERSONNEL MANAGEMENT

5.1 Medical Services

5.2 Drug Screening and Alcohol Testing


All Jacobs personnel are subject to the provisions of the Jacobs Drug Screening and
Alcohol Testing policy along with Lima Husky Refinery requirements.

5.3 Training and Certification


The Lima Husky Refinery safety orientation along with the Jacobs project-specific
orientation will be completed by all Jacobs personnel prior to starting work at the site.
Training for specific tasks will be required. This includes, but is not limited to the following:
Defensive Driving
Ladders/Stairs
Fall Prevention and Protection (if applicable)
Safe Plan of Action (SPA)
Safety Observation Report (SOR)
Confined Space (if applicable)
Hazard Communication (hydrogen sulfide, benzene)
Signs and Tags
Personal Protective Equipment
Additional training will be organized and conducted as needed.

6.0 HSE PROGRAM

6.1 HSE Program Implementation


Core HSE program elements shall be as follows:
All tasks shall require the development of a Safe Plan of Action (SPA) at the
location where the task is performed. All personnel working on the task must review
and sign the SPA.
All project personnel are expected to participate in the Safety Observation
process.
6.2 Work Control
The following work permits will be issued by Lima Husky Refinery and utilized at the site.
Applicable training will be provided. Permits include, but are not limited to:
Safe Work Permit
Vehicle Entry Permit (if applicable)
Confined Space Entry Permit (if applicable)
6.3 PPE and Clothing Requirements
The following are PPE and clothing requirements when working in process areas or labs.
Additional requirements are defined in the Lima Husky Refinery Contractor and Personal
Protective Equipment Policy and Jacobs HSEPs.
Hardhats (ANSI Z89)
Hardhats will be worn with the bill forward.

Company and employee names will be clearly legible and visible on the front of
the hardhat.
Eye and Face Protection (ANSI Z87.1)
Safety glasses with rigid side shields. This includes both prescription and non-
prescription eyewear.

Clear safety glasses shall be worn inside all buildings.


When goggles are required, these will be worn around the head and not the
hardhat.
Contact lenses are allowed in plant operating areas when worn with appropriate
eye and/or face protection.

Foot Protection (ANSI Z41 or ASTM F2412-05 & F2413-05)


Safety toe footwear (steel or composite) of leather or equivalent material.
Footwear must be able to resist liquid penetration and/or absorption.
Well-defined heel not less than high.

Hearing Protection
Must be a minimum NRR of 25 dB (decibels)
In posted areas (Hearing Protection Required In This Area and Hearing
Protection Required When Equipment Is Operating)
In areas where noise levels are such that one must raise their voice to be heard
when standing next to another employee.

Clothing and Attire


Long-sleeved fire retardant clothing (FRC)
Buttoned at the sleeve and zipped or buttoned at the neck.
Will be the outermost garment (unless a rain suit is required)

Gloves
Leather gloves

6.4 Unattended Vehicles


Vehicles are not to be left running.
Keys are to remain in the vehicle within the battery limits of a process unit.

6.5 Smoking Policy


Smoking is only allowed in designated areas.
Strike anywhere kitchen matches and/or lighters with an exposed striking mechanism are
not allowed.

6.6 Cell Phones


Cell phones are not allowed in any process unit unless a Hot Work Permit has been
obtained.

6.7 Facial Hair


Moustaches must not extend below the corners of the mouth.
Side burns must not extend below the ear lobes.
Facial hair below the lip must be trimmed and should not interfere with the function of the
respirator should one be required.

6.8 Personal H2S Monitors


H2S monitors are to be worn by all personnel in operating areas.

6.9 Incident Reporting and Investigation


All incidents that involve treatment of an injury by a medical professional, a near miss,
equipment and/or property damage, motor vehicle incidents and business interruption will
be reported and investigated.
All cases shall be investigated the same day of the incident or approved by the Jacobs
Area HSE Manager to be held on another day. No investigation shall lapse more than 24
hours of the incident.

7.0 HAZARD ANALYSIS


The hazard analysis process is a collaborative effort involving personnel with the appropriate
expertise to ensure all credible hazards are identified and addressed.
The hazard analysis has been documented as part of the HASAP. The hazard analysis will be
periodically reviewed and updated to ensure that it is maintained current with the scope of the
work and the hazards that exist. Revisions to the hazard analysis resulting in changes in risk
assessment, hazards and/or controls will be communicated to the project team as appropriate.
The hazard analysis has been performed and is provided as Attachment A.

8.0 SITE ACCESS AND CONTROL, VEHICLE SAFETY AND SECURITY

8.1 Access and Control


Access and control to the physical work site is through the main gate at Metcalf Road.

8.2 Security Measures


No unauthorized devices (cameras, computers, etc.) are allowed within the Lima Husky
Refinery without prior written consent.

8.3 Traffic Plan and Vehicular Safety


Personnel driving on site are subject to all Lima Husky Refinery rules and regulations.
Proper PPE must be worn at all times while outside of a vehicle and within the jobsite
perimeter. Seat belts and other factory restraint devices must be worn. Riding in the back
of trucks is prohibited.
All Jacobs site personnel shall complete the Jacobs sponsored Defensive Driving Course
or equivalent prior to operating a vehicle on company business.

9.2 General Emergency Response


The Lima Husky Refinery has an extensive emergency response plan which may be
initiated in the event of an emergency. Emergencies may include fire and/or gas release,
serious injuries, chemical release, spill or air emission, national emergency or other non-
emergencies such as power outages, chemical releases from outside the plant or
impending severe weather conditions.
Evacuation routes, roll call/staging areas and shelter-in-place locations and maps have
been developed and identified by the Lima Husky Refinery personnel.

9.3 Medical Emergency Response


Non-life threatening medical emergencies should be transported to the occupational clinic
during normal working hours (7:00 AM to 5:00 PM weekdays) and to the designated
hospital emergency room during off hours.
Serious and/or life threatening medical emergencies shall be routed through Lima Husky
Refinery Security.

10.0 HSE RECORDS


Project records will be maintained at the Jacobs Cincinnati office. Records shall include, but are
not limited to:
