I have the honor to refer to your Office, the records of investigation relative to the case of
Reckless Imprudence in Damage to Property(Republic act #1790), where the person to be
charge Jonas Babierra, 28, resident in Molino II Bacoor City, Cavite.

COMPLAINANT:
Mr. Leo Carpio, 45 years old, married, and a resident of Molino 1, Bacoor City

EVIDENCE/S:
1. Sworn statement of the witnesses.
2. Sworn statement of the complainant.

DATE/TIME/PLACE OFF OCCURRENCE:

23 September 2014/ 10.30 PM/ Tuesday while driving at the Brgy. Molino 1
few meters away from the house of the victim.
NARRATION OF THE CASE:

On 23 September 2014, Mr. Leo Carpio (was the witnesses the incident of his house).
The complainant was seen the crash of his house at about 10:30 pm in the evening that are totally
broken and no one injured in that incident.

The complainant checks the driver if he is alive or dead and the complainant call a police
and ambulance. Then the investigation the car and the driver were on progress.

The chief investigator was check the but it seen like no problem of the car and then the
police officers was check the driver and ask some question the accident happen. So the police
officers talk to the witnesses about the accident happen. The 2(two) witnesses was said that the
driver was so fast and it midnight while no streetlight in the Molino road.

Then the police officers were taking the driver in his offense and direct to the police
headquarters. This case will be presented in court by Insp. Reynaldo C. Aquino PNP.

SWORN STATEMENT OF COMPLAINANT

I. I Leo Carpio, Filipino, Of Legal Age, and with the residence at Molino 1 Bacoor
City, Cavite. At about 10:30 pm in the evening, 23 September 2014. I smoking
outside in my house and there is an accident that happen in my house were Im
outside of my house. There was a car that was so fast coming to my house. Then I run
fast to the incident because the responded was really bad and critical condition. And
then I call ambulance and I report to the police what happen to my house.

SWORN STATEMENT OF WITNESS

I. I Mac Mac Ventura, Filipino, of legal age, and residence of Molino 1 Bacoor City
Cavite. At about 10:20 pm in the evening, 23 September 2014. I was walking alone
and I m listening music at my phone. I see the car that coming so fast that something
wrong in the car. The car was so fast coming into the house. The car was collide
against in the gate and continuous to the house. I was shock and a few minutes I saw
the owner of the house, ran to the respondent and a few minutes again the ambulance
was arrived to the area. The police was goes well. And the police officers were taking
charge to the area.
